The PCI-DMA-mapping.txt moved to general docs and became DMA-API-HOWTO.txt
in 5e07c2c7301b ("Documentation: rename PCI/PCI-DMA-mapping.txt to
DMA-API-HOWTO.txt").  Add new file about PCI Express I/O Virtualization.

Create/use more directory structure in the Documentation/ tree.

Create Documentation/blockdev/ sub-directory and populate it.
Populate the Documentation/serial/ sub-directory.
Move MSI-HOWTO.txt to Documentation/PCI/.
Move ioctl-number.txt to Documentation/ioctl/.

Update all relevant Kconfig files and source files.
