Fix a regression that has caused accesses to the PCI MMIO window to
complete unclaimed in non-EVA configurations with the SOC-it family of
system controllers, preventing PCI devices from working that use MMIO.

In the non-EVA case PHYS_OFFSET is set to 0, meaning that PCI_BAR0 is
set with an empty mask (and PCI_HEAD4 matches addresses starting from 0
accordingly).  Consequently all addresses are matched for incoming DMA
accesses from PCI.  This seems to confuse the system controller's logic
and outgoing bus cycles targeting the PCI MMIO window seem not to make
it to the intended devices.

This happens as well when a wider mask is used with PCI_BAR0, such as
0x80000000 or 0xe0000000, that makes addresses match that overlap with
the PCI MMIO window, which starts at 0x10000000 in our configuration.

Set the mask in PCI_BAR0 to 0xf0000000 for non-EVA then, covering the
non-EVA maximum 256 MiB of RAM, which is what YAMON does and which used
to work correctly up to the offending commit.  Set PCI_P2SCMSKL to match
PCI_BAR0 as required by the system controller's specification, and match
PCI_P2SCMAPL to PCI_HEAD4 for identity mapping.

Covering the PCI southbridge legacy port I/O range with a northbridge
resource reservation prevents MIPS Malta platform code from claiming its
standard legacy resources.  This is because request_resource() calls
cause a clash with the previous reservation and consequently fail.

Change to using insert_resource() so as to prevent the clash, switching
the legacy reservations from:

MIPS Malta platform code registers the PCI southbridge legacy port I/O
PS/2 keyboard range as a standard resource marked as busy.  It prevents
the i8042 driver from registering as it fails to claim the resource in
a call to i8042_platform_init().  Consequently PS/2 keyboard and mouse
devices cannot be used with this platform.

Fix the issue by removing the busy marker from the standard reservation,
making the driver register successfully:

  serio: i8042 KBD port at 0x60,0x64 irq 1
  serio: i8042 AUX port at 0x60,0x64 irq 12

and the resource show up as expected among the legacy devices:

To prepare for completely separating the VGA console screen_info from
the one used in EFI/sysfb, rename the vgacon instances and make them
local as much as possible.

ia64 and arm both have confurations with vgacon and efi, but the contents
never overlaps because ia64 has no EFI framebuffer, and arm only has
vga console on legacy platforms without EFI. Renaming these is required
before the EFI screen_info can be moved into drivers/firmware.

The ia64 vga console is actually registered in two places from
setup_arch(), but one of them is wrong, so drop the one in pcdp.c and
fix the one in setup.c to use the correct conditional.

x86 has to keep them together, as the boot protocol is used to switch
between VGA text console and framebuffer through the screen_info data.

Starting with following patch MIPS Malta is not able to boot:

The reason is the alignment test added to the fdt_ro_probe_(). To fix
this issue, we need to make sure that fdt_buf is aligned.

Since the dtc patch was designed to uncover potential issue, I handle
initial MIPS Malta patch as initial bug.

<title>MIPS: Malta: Do not byte-swap accesses to the CBUS UART</title>

Correct big-endian accesses to the CBUS UART, a Malta on-board discrete
TI16C550C part wired directly to the system controller's device bus, and
do not use byte swapping with the 32-bit accesses to the device.

The CBUS is used for devices such as the boot flash memory needed early
on in system bootstrap even before PCI has been initialised.  Therefore
it uses the system controller's device bus, which follows the endianness
set with the CPU, which means no byte-swapping is ever required for data
accesses to CBUS, unlike with PCI.

The CBUS UART uses the UPIO_MEM32 access method, that is the `readl' and
`writel' MMIO accessors, which on the MIPS platform imply byte-swapping
with PCI systems.  Consequently the wrong byte lane is accessed with the
big-endian configuration and the UART is not correctly accessed.

As it happens the UPIO_MEM32BE access method makes use of the `ioread32'
and `iowrite32' MMIO accessors, which still use `readl' and `writel'
respectively, however they byte-swap data passed, effectively cancelling
swapping done with the accessors themselves and making it suitable for
the CBUS UART.

Make the CBUS UART switch between UPIO_MEM32 and UPIO_MEM32BE then,
based on the endianness selected.  With this change in place the device
is correctly recognised with big-endian Malta at boot, along with the
Super I/O devices behind PCI:

Serial: 8250/16550 driver, 5 ports, IRQ sharing enabled
printk: console [ttyS0] disabled
serial8250.0: ttyS0 at I/O 0x3f8 (irq = 4, base_baud = 115200) is a 16550A
