As of commit dd472da38, rwsem.h was moved into asm-generic.
This patch removes the arch file and points the build at
its new location.

Add swab.h to kbuild.asm and remove the individual entries from
each arch, mark as unifdef as some arches have some kernel-only
bits inside.

git mv include/asm-powerpc/* arch/powerpc/include/asm

Followed by a few documentation/comment fixups and a couple of places
where &lt;asm-powepc/...&gt; was being used explicitly.  Of the latter only
one was outside the arch code and it is a driver only built for powerpc.
