powerpc/isa-bridge: Fix ISA mapping when "ranges" is not present

Commit e4ab08be5b49 ("powerpc/isa-bridge: Remove open coded "ranges"
parsing") broke PASemi Nemo board booting. The issue is the ISA I/O
range was not getting mapped as the logic to handle no "ranges" was
inverted. If phb_io_base_phys is non-zero, then the ISA range defaults
to the first 64K of the PCI I/O space. phb_io_base_phys should only be 0
when looking for a non-PCI ISA region.

powerpc/isa-bridge: Remove open coded "ranges" parsing

"ranges" is a standard property with common parsing functions. Users
shouldn't be implementing their own parsing of it. Reimplement the
ISA brige "ranges" parsing using the common ranges iterator functions.

The common routines are flexible enough to work on PCI and non-PCI to
ISA bridges, so refactor pci_process_ISA_OF_ranges() and
isa_bridge_init_non_pci() into a single implementation.

Don't inherit headers "by chances" from asm/prom.h, asm/mpc52xx.h,
asm/pci.h etc...

Include the needed headers, and remove asm/prom.h when it was
needed exclusively for pulling necessary headers.

Move the main API comment from the _noflush variant to the normal
variant, and make _noflush internal to mm/.

powerpc: remove __ioremap_at and __iounmap_at

These helpers are only used for remapping the ISA I/O base.  Replace the
mapping side with a remap_isa_range helper in isa-bridge.c that hard codes
all the known arguments, and just remove __iounmap_at in favour of open
coding it in the only caller.

powerpc: Use device_type helpers to access the node type

Remove directly accessing device_node.type pointer and use the
accessors instead. This will eventually allow removing the type
pointer.

Replace the open coded iterating over child nodes with
for_each_child_of_node() while we're here.

powerpc/mm: properly set PAGE_KERNEL flags in ioremap()

Set PAGE_KERNEL directly in the caller and do not rely on a
hack adding PAGE_KERNEL flags when _PAGE_PRESENT is not set.

As already done for PPC64, use pgprot_cache() helpers instead of
_PAGE_XXX flags in PPC32 ioremap() derived functions.
