<title>powerpc: Hardware breakpoints rewrite to handle non DABR breakpoint registers</title>

This is a rewrite so that we don't assume we are using the DABR throughout the
code. We now use the arch_hw_breakpoint to store the breakpoint in a generic
manner in the thread_struct, rather than storing the raw DABR value.

The ptrace GET/SET_DEBUGREG interface currently passes the raw DABR in from
userspace. We keep this functionality, so that future changes (like the POWER8
DAWR), will still fake the DABR to userspace.

PowerPC has non standard getregs calls that only dump the GPRs or
FPRs and have their arguments reversed. commit e17666ba48f7 (ptrace
updates &amp; new, better requests) in 2.6.3 deprecated them and introduced
more standard versions.

It's been about 5 years and I know of no users of the old calls so
lets remove them.

Update compat_arch_ptrace to follow recent changes in
PTRACE_GET_DEBUGREG and the addition of
PPC_PTRACE_{GETHWDBGINFO|{SET|DEL}HWDEBUG}. The latter three can be

* Remove smp_lock.h from files which don't need it (including some headers!)
* Add smp_lock.h to files which do need it
* Make smp_lock.h include conditional in hardirq.h
It's needed only for one kernel_locked() usage which is under CONFIG_PREEMPT

This will make hardirq.h inclusion cheaper for every PREEMPT=n config
  (which includes allmodconfig/allyesconfig, BTW)

<title>powerpc: Fix ptrace compat wrapper for FPU register access</title>

The ptrace compat wrapper mishandles access to the fpu registers. The
PTRACE_PEEKUSR and PTRACE_POKEUSR requests miscalculate the index into
the fpr array due to the broken FPINDEX macro. The
PPC_PTRACE_PEEKUSR_3264 request needs to use the same formula that the
native ptrace interface uses when operating on the register number (as
opposed to the 4-byte offset). The PPC_PTRACE_POKEUSR_3264 request
didn't take TS_FPRWIDTH into account.

We are going to change where the floating point registers are stored
in the thread_struct, so in preparation add some macros to access the
floating point registers. Update all code to use these new macros.

This reverts commit e4cc58944c1e2ce41e3079d4eb60c95e7ce04b2b, as
requested by Roland McGrath, because compat_ptrace_request (added in
commit e16b27816462de700f9508d86954410c41105dc2, "ptrace:

Current versions of gdb require a working implementation of
PTRACE_GETSIGINFO for proper watchpoint support. Since struct siginfo
contains pointers it must be converted when passed to a 32-bit debugger.
