Our SMP cache flush ops use CPU cross calls to deal with things
like I-cache accesses not being broadcast in hardware, so ensure that
the CACHE_FLUSH_IS_SAFE reflects this.

<title>kgdbts: unify/generalize gdb breakpoint adjustment</title>

The Blackfin arch, like the x86 arch, needs to adjust the PC manually
after a breakpoint is hit as normally this is handled by the remote gdb.
However, rather than starting another arch ifdef mess, create a common
GDB_ADJUSTS_BREAK_OFFSET define for any arch to opt-in via their kgdb.h.

The kgdb stub has traditionally tied in to the NMI slot, and manually
handled debounce. Now that we have a generic way to do this instead, all
of the stub-specific debounce silliness can be killed off.

This migrates from the old bitrotted kgdb stub implementation and moves
to the generic stub. In the process support for SH-2/SH-2A is also added,
which the old stub never provided.

Most of the moving about was done with Sam's directions at:
