<title>x86: replace futex_atomic_cmpxchg_inatomic() with user_atomic_cmpxchg_inatomic</title>

futex_atomic_cmpxchg_inatomic() is simply the 32-bit implementation of
user_atomic_cmpxchg_inatomic(), which in turn is simply a
generalization of the original code in
futex_atomic_cmpxchg_inatomic().

Use the newly generalized user_atomic_cmpxchg_inatomic() as the futex
implementation, too.

<title>x86, smap: Add STAC and CLAC instructions to control user space access</title>

When Supervisor Mode Access Prevention (SMAP) is enabled, access to
userspace from the kernel is controlled by the AC flag. To make the
performance of manipulating that flag acceptable, there are two new
instructions, STAC and CLAC, to set and clear it.

This patch adds those instructions, via alternative(), when the SMAP
feature is enabled. It also adds X86_EFLAGS_AC unconditionally to the
SYSCALL entry mask; there is simply no reason to make that one
conditional.

Change futex_atomic_op_inuser and futex_atomic_cmpxchg_inatomic
prototypes to use u32 types for the futex as this is the data type the
futex core code uses all over the place.

<title>futex: Sanitize cmpxchg_futex_value_locked API</title>

The cmpxchg_futex_value_locked API was funny in that it returned either
the original, user-exposed futex value OR an error code such as -EFAULT.
This was confusing at best, and could be a source of livelocks in places
that retry the cmpxchg_futex_value_locked after trying to fix the issue
by running fault_in_user_writeable().

This change makes the cmpxchg_futex_value_locked API more similar to the
get_futex_value_locked one, returning an error code and updating the
original value through a reference argument.

Change header guards named "ASM_X86__*" to "_ASM_X86_*" since:

a. the double underscore is ugly and pointless.
b. no leading underscore violates namespace constraints.
