xtensa: add asm-prototypes.h

Move assembly source prototypes from xtensa_ksyms.c to
asm/asm-prototypes.h, move corresponding EXPORT_SYMBOLs to the assembly
sources and enable HAVE_ASM_MODVERSIONS for xtensa.

xtensa: simplify coherent_kvaddr logic

Functions coherent_kvaddr, clear_page_alias and copy_page_alias use
physical address 0 as a special value that means 'this page is in the
KSEG mapping and its existing virtual address has the same color as the
virtual address of its future mapping, so don't map it to the
TLBTEMP_BASE area'.

Simplify this logic and drop special handling of low memory pages/pages
with coherent mapping and always use TLBTEMP_BASE area.

mm: introduce include/linux/pgtable.h

manipulation functions.

Start with moving asm-generic/pgtable.h to include/linux/pgtable.h and
make the latter include asm/pgtable.h.

xtensa: abstract 'entry' and 'retw' in assembly code

Provide abi_entry, abi_entry_default, abi_ret and abi_ret_default macros
that allocate aligned stack frame in windowed and call0 ABIs.
Provide XTENSA_SPILL_STACK_RESERVE macro that specifies required stack
frame size when register spilling is involved.
Replace all uses of 'entry' and 'retw' with the above macros.
This makes most of the xtensa assembly code ready for XEA3 and call0 ABI.

xtensa: implement clear_user_highpage and copy_user_highpage

Existing clear_user_page and copy_user_page cannot be used with highmem
because they calculate physical page address from its virtual address
and do it incorrectly in case of high memory page mapped with
kmap_atomic. Also kmap is not needed, as most likely userspace mapping
color would be different from the kmapped color.

Provide clear_user_highpage and copy_user_highpage functions that
determine if temporary mapping is needed for the pages. Move most of the
logic of the former clear_user_page and copy_user_page to
xtensa/mm/cache.c only leaving temporary mapping setup, invalidation and
clearing/copying in the xtensa/mm/misc.S. Rename these functions to
clear_page_alias and copy_page_alias.
