Use separate gendisks (which share a tag_set) for the native Amgiga vs
the MS-DOS mode instead of redirecting the gendisk lookup using a probe
callback.  This avoids potential problems with aliased block_device
instances and will eventually allow for removing the blk_register_region
framework.

Add SPDX license identifiers to all files which:

 - Have no license information of any form

 - Have MODULE_LICENCE("GPL*") inside which was used in the initial
   scan/conversion to ignore the file

These files fall under the project license, GPL v2 only. The resulting SPDX
license identifier is:

  GPL-2.0-only

This reverts commit 9fd097b14918875bd6f125ed699d7bbbba5893ee.

Instead of leaving disk->events completely empty, we now export the
supported events again, and tell the block layer not to forward events to
user space by not setting DISK_EVENT_FLAG_UEVENT. This allows the block
layer to distinguish between devices that for which events should be
handled in kernel only, and devices which don't support any meda change
events at all.

amifd.h and amifdreg.h are only used from amiflop.c, and they're pretty
small, so move the contents to amiflop.c and get rid of the .h files.
This is preparation for adding a struct blk_mq_tag_set to struct
amiga_floppy_struct.

This converts the amifloppy driver to pass the timer pointer to the
callback instead of the drive number (and flags). It eliminates the
decusagecounter flag, as it was unused, and drops the ininterrupt flag
which appeared to be a needless optimization. The drive can then be
calculated from the offset of the timer in the drive timer array.

Additionally moves to a static data variable instead of the
soon-to-be-gone timer->data field.
