block: remove the gendisk argument to blk_execute_rq

Remove the gendisk aregument to blk_execute_rq and blk_execute_rq_nowait
given that it is unused now.  Also convert the boolean at_head parameter
to actually use the bool type while touching the prototype.

block/sx8: add error handling support for add_disk()

We never checked for errors on add_disk() as this function
returned void. Now that this is fixed, use the shiny new
error handling.

A completion is used to notify the initial probe what is
happening and so we must defer error handling on completion.
Do this by remembering the error and using the shared cleanup
function.

The tags are shared and so are hanlded later for the
driver already.

The 'q' is not used since commit a1ce35fa4985 ("block: remove dead
elevator code"), also update the comment of the function.

And more importantly it never really was needed to start with given
that we can trivial derive it from struct request.

sx8: use a per-host tag_set

The current sx8 code spends a lot of effort dealing with the fact that
tags are per-host, but there might be multiple queues.  Now that the
driver has been converted to blk-mq it can take care of the blk-mq
tag_set concept that has been designed just for that.
