<title>dmaengine: Replace WARN_TAINT_ONCE() with pr_warn_once()</title>

The WARN_TAINT_ONCE() prints out a loud stack trace on broken BIOSes.
The systems that have this problem are several years out of support and
no longer have BIOS updates available. The stack trace isn't necessary
and a pr_warn_once() will do.

Change WARN_TAINT_ONCE() to pr_warn_once() and taint.

Includes an appropriate header file dma_v2.h in ioat/dca.c because
functions ioat2_dca_init() and ioat3_dca_init() have their function
declarations in dma_v2.h.

This eliminates the following warning in ioat/dca.c:
drivers/dma/ioat/dca.c:410:22: warning: no previous prototype for ‘ioat2_dca_init’ [-Wmissing-prototypes]
drivers/dma/ioat/dca.c:624:22: warning: no previous prototype for ‘ioat3_dca_init’ [-Wmissing-prototypes]

<title>ioat/dca: Update DCA BIOS workarounds to use TAINT_FIRMWARE_WORKAROUND</title>

<id>f3c78f8515dd4cb9e26040ebb54cf589d21137ee</id>

This patch is meant to be a follow-up for a patch originally submitted under
the title "ioat: Do not enable DCA if tag map is invalid". It was brought to
my attention that the preferred approach for BIOS workarounds is to set the
taint flag for TAINT_FIRMWARE_WORKAROUND for systems that require BIOS
workarounds.

This change makes it so that the DCA workarounds for broken BIOSes will now
use WARN_TAINT_ONCE(1, TAINT_FIRMWARE_WORKAROUND, ...) instead of just
printing a message via dev_err.
