This patch updates the code parsing the "jedec,lpddr2" device tree
binding to use the new `revision-id` property instead of the deprecated
`revision-id1` and `revision-id2` properties if available.

Passing the memory timings maximum frequency as an unit address was
a workaround and instead 'max-freq' is preferred. Look for 'max-freq'
first and then fallback to 'reg'.

Messages printed by generic of_memory code will still be using device
context so their location/meaning will be known. Printing __func__ is
not needed.

Add AC timings information needed to support LPDDR3 and memory
controllers along with helpers to obtain it. These will be necessary
for upcoming Exynos5422 Dynamic Memory Controller driver.

Now that jedec_ddr_data.c was moved from lib/ to drivers/memory/,
<memory/jedec_ddr.h> is included only from drivers/memory/.

Make it a local header of drivers/memory/.

The directory include/memory is now gone.

While I am here, I also changed #include <linux/module.h> to
<linux/export.h>. Because CONFIG_DDR is bool, jedec_ddr_data.c is
never compiled as a module.
