This patch removes the MIC drivers from the kernel tree
since the corresponding devices have been discontinued.

Removing the dma and char-misc changes in one patch and
merging via the char-misc tree is best to avoid any
potential build breakage.

Split out all the bits that are purely for dma_map_ops implementations
and related code into a new &lt;linux/dma-map-ops.h&gt; header so that they
don't get pulled into all the drivers.  That also means the architecture
specific &lt;asm/dma-mapping.h&gt; is not pulled in by &lt;linux/dma-mapping.h&gt;
any more, which leads to a missing includes that were pulled in by the
x86 or arm versions in a few not overly portable drivers.

The correct format is not used and no attempt has been made
to document the function arguments.  Makes sense to just demote
the headers back down to simple comments.

mic_x100_intr_init[] is only ever used in mic_x100.c, so instead
of listing to the compiler complain about unused static arrays
simply move it into the c-file for direct consumption.  This way
it can stay 'static'.

Since snprintf() returns the would-be-output size instead of the
actual output size, the succeeding calls may go beyond the given
buffer limit.  Fix it by replacing with scnprintf().
