<title>USB: convert some miscellanies drivers to use module_usb_driver()</title>

This converts the remaining USB drivers in the kernel to use the
module_usb_driver() macro which makes the code smaller and a bit
simpler.

Added bonus is that it removes some unneeded kernel log messages about
drivers loading and/or unloading.

Add a driver for USB SD Host Controller devices.  These devices are
Cypress Astoria chips with firmware compliant with issue 2 of CSR's USHC
specification.
