breaks, incorrect separators, and some name mismatches.  Delete

<title>drivers/net: delete all code/drivers depending on CONFIG_MCA</title>

The support for CONFIG_MCA is being removed, since the 20
year old hardware simply isn't capable of meeting today's
software demands on CPU and memory resources.

This commit removes any MCA specific net drivers, and removes
any MCA specific probe/support code from drivers that were
doing a dual ISA/MCA role.

There's no need to test for a NULL pointer before calling
release_firmware() since the function does that check itself, so
remove the redundant test.

Use eth_mac_addr() for .ndo_set_mac_address, remove
typhoon_set_mac_address() since it do currently the same as
eth_mac_addr(). Additional advantage: eth_mac_addr() already
checks if the given address is valid.

Jean Delvare reported bonding on top of 3c59x adapters was not detecting
network cable removal fast enough.

3c59x indeed uses a 60 seconds timer to check link status if carrier is
on, and 5 seconds if carrier is off.

This patch reduces timer period to 5 seconds if device is a bonding
slave.
