dev_consume_skb_irq() should be called in amd8111e_tx() when xmit
done. It makes drop profiles more friendly.

The XGBE hardware has support for performing MDIO operations using an
MDIO command request. The driver mistakenly uses the mdio port address
as the MDIO command request device address instead of the MDIO command
request port address. Additionally, the driver does not properly check
for and create a clause 45 MDIO command.

Check the supplied MDIO register to determine if the request is a clause
45 operation (MII_ADDR_C45). For a clause 45 operation, extract the device
address and register number from the supplied MDIO register and use them
to set the MDIO command request device address and register number fields.
For a clause 22 operation, the MDIO request device address is set to zero
and the MDIO command request register number is set to the supplied MDIO
register. In either case, the supplied MDIO port address is used as the
MDIO command request port address.

After phy_start() phylib takes care of all needed actions, including
aneg settings and checking link state. There's no need to set state
PHY_CHANGELINK in drivers.

Convert string compares of DT node names to use of_node_name_eq helper
instead. This removes direct access to the node name pointer.

For instances using of_node_cmp, this has the side effect of now using
case sensitive comparisons. This should not matter for any FDT based
system which all of these are.

of_find_node_by_path() acquires a reference to the node
returned by it and that reference needs to be dropped by its caller.
This place doesn't do that, so fix it.

There are a few MAC/PHYs combinations which now support > 1Gbps. These
may need to make use of link modes with bits > 31. Thus their
supported PHY features or advertised features cannot be implemented
using the current bitmap in a u32. Convert to using a linkmode bitmap,
which can support all the currently devices link modes, and is future
proof as more modes are added.

Drivers using generic NAPI interface no longer need to include
<net/busy_poll.h>, since busy polling was moved to core networking
stack long ago.

Fix a commit 4bcc595ccd80 ("printk: reinstate KERN_CONT for printing
continuation lines") regression with the `declance' driver, which caused
the adapter identification message to be split between two lines, e.g.:

declance.c: v0.011 by Linux MIPS DECstation task force
tc6: PMAD-AA
, addr = 08:00:2b:1b:2a:6a, irq = 14
tc6: registered as eth0.

Address that properly, by printing identification with a single call,
making the messages now look like:

declance.c: v0.011 by Linux MIPS DECstation task force
tc6: PMAD-AA, addr = 08:00:2b:1b:2a:6a, irq = 14
tc6: registered as eth0.
