bna: prevent IOC timer rearm during teardown

bnad_pci_remove() and the probe disable_ioceth path call
timer_delete_sync() for ioc_timer, sem_timer and hb_timer, but not for
iocpf_timer.  bnad_iocpf_timeout() then takes bnad-&gt;bna_lock after
free_netdev() has freed the struct bnad.

Deleting iocpf_timer last does not fix this.  sem_timer and
iocpf_timer rearm each other: bnad_iocpf_sem_timeout() can arm
iocpf_timer, and bnad_iocpf_timeout() arms sem_timer from
bfa_ioc_hw_sem_get() when the semaphore is busy.
timer_delete_sync() only waits out its own callback.
bnad_ioceth_disable() can time out and leave that callback live.

Shut all four IOC timers down with timer_shutdown_sync() on both
paths, so a later mod_timer() is ignored.

This issue was identified during our ongoing static-analysis research
while reviewing kernel code.

If dma_map_single() fails in bnad_start_xmit(), the skb is freed, but
head_unmap-&gt;skb was set before the mapping attempt and is not cleared. The
producer index is not advanced, so later transmissions normally overwrite
the entry.

However, if the interface is brought down first, bnad_txq_cleanup() scans
the entire unmap queue, finds the stale pointer, and calls
bnad_tx_buff_unmap() on it. That function dereferences the freed skb in
skb_headlen(). Its zero nvecs count is decremented to -1, causing its
while (nvecs) loop to repeatedly unmap entries around the TX ring and
potentially hang cleanup.

Set head_unmap-&gt;skb after the first DMA mapping succeeds. This prevents the
stale entry from reaching bnad_tx_buff_unmap().

This converts some of the visually simpler cases that have been split
over multiple lines.  I only did the ones that are easy to verify the
resulting diff by having just that final GFP_KERNEL argument on the next
line.

Somebody should probably do a proper coccinelle script for this, but for
me the trivial script actually resulted in an assertion failure in the
middle of the script.  I probably had made it a bit _too_ trivial.

So after fighting that far a while I decided to just do some of the
syntactically simpler cases with variations of the previous 'sed'
scripts.

The more syntactically complex multi-line cases would mostly really want
whitespace cleanup anyway.

This was done entirely with mindless brute force, using

    git grep -l '\&lt;k[vmz]*alloc_objs*(.*, GFP_KERNEL)' |
        xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/'

to convert the new alloc_obj() users that had a simple GFP_KERNEL

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:

In fact, these two are not tied closely to each other. The only
requirements to GRO are to use it in the BH context and have some
sane limits on the packet batches, e.g. NAPI has a limit of its
budget (64/8/etc.).
Move purely GRO fields into a new structure, &amp;gro_node. Embed it
into &amp;napi_struct and adjust all the references.
gro_node::cached_napi_id is effectively the same as
napi_struct::napi_id, but to be used on GRO hotpath to mark skbs.
napi_struct::napi_id is now a fully control path field.

Three Ethernet drivers use napi_gro_flush() not really meant to be
exported, so move it to &lt;net/gro.h&gt; and add that include there.
napi_gro_receive() is used in more than 100 drivers, keep it
in &lt;linux/netdevice.h&gt;.
This does not make GRO ready to use outside of the NAPI context
yet.
