The TX DMA engine issues upstream read requests when there is room in
the TX FIFO for the completion. However, the fetches for the rest of
the packet might be delayed by any back pressure.  Since a flush must
wait for an EOP, the entire flush may be delayed by back pressure.

Mitigate this by disabling flow control before the flushes are
started.  Since PF and VF flushes run in parallel introduce
fc_disable, a reference count of the number of flushes outstanding.

The same principle could be applied to Falcon, but that
would bring with it its own testing.

We sometimes hit a "failed to flush" timeout on some TX queues, but the
flushes have completed and the flush completion events seem to go missing.
In this case, we can check the TX_DESC_PTR_TBL register and drain the
queues if the flushes had finished.

The phy, mac, and board information structures should be const.
Since tables contain function pointer this improves security
(at least theoretically).

Compile tested only.

Replace EFX_ERR() with netif_err(), EFX_INFO() with netif_info(),
EFX_LOG() with netif_dbg() and EFX_TRACE() and EFX_REGDUMP() with
netif_vdbg().

Replace EFX_ERR_RL(), EFX_INFO_RL() and EFX_LOG_RL() using explicit
calls to net_ratelimit().

Implement the ethtool operations to get and set message level flags,
and add a 'debug' module parameter for the initial value.

When set, an event is not sent whenever periodic MAC statistics are
raised.  This avoids unnecessary wake-ups.

This adds support for the SFC9000 family of 10G Ethernet controllers
and LAN-on-motherboard chips, starting with the SFL9021 'Siena' and
SFC9020 'Bethpage'.

The SFC9000 family is based on the SFC4000 'Falcon' architecture, but
with some significant changes:

- Two ports are associated with two independent PCI functions
  (except SFC9010)
- Integrated 10GBASE-T PHY(s) (SFL9021/9022)
- MAC, PHY and board peripherals are managed by firmware
  - Driver does not require board-specific code
  - Firmware supports wake-on-LAN and lights-out management through NC-SI
- IPv6 checksum offload and RSS
- Filtering by MAC address and VLAN (not included in this code)
- PCI SR-IOV (not included in this code)
