 - use be32_to_cpu() instead of ntohs() for 32 bit port capabilities.

 - add a new function fwcaps32_to_caps16() to convert 32 bit port
   capabilities to 16 bit port capabilities.

- Updates csio_get_flash_params() to take care of ISSI, Macronix and
  Winbond FLASH parts.

- Assume flash part size to be 4MB if it cannot be identified

T5/T6 can have different pack and pad boundary value. This patch sets
packing boundary based on cache line size and PCI-E maximum payload size
and sets smallest padding boundary value.

The kzalloc() function has a 2-factor argument form, kcalloc(). This
patch replaces cases of:

        kzalloc(a * b, gfp)

with:
        kcalloc(a * b, gfp)

as well as handling cases of:

        kzalloc(a * b * c, gfp)

with:

        kzalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kzalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kzalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

The Coccinelle script used for this was:

MDI Port Capabilities bit definitions were inconsistent with
regard to the MDI enum values. 2 bits used to define MDI in
the port capabilities are not really separable, it's a 2-bit
field with 4 different values. Change the port capability bit
definitions to be "AUTO" and "STRAIGHT" in order to get them
to line up with the enum's.

qla2xxx: Fix Async GPN_FT for FCP and FC-NVMe scan" and "2b5b96473efc
scsi: qla2xxx: Fix FC-NVMe LUN discovery"

Merge is non-trivial and has been verified by Qlogic (Cavium)

The pointer ln is assigned a value that is never read, it is re-assigned
a new value in the list_for_each loop hence the initialization is
redundant and can be removed.

Cleans up clang warning:
