This patch changes the !blk-mq path to the same defaults as the blk-mq
I/O path by always enabling block tagging, and always using host wide
tags.  We've had blk-mq available for a few releases so bugs with
this mode should have been ironed out, and this ensures we get better
coverage of over tagging setup over different configs.

Make PCI Device ID Tables be "const" to move them out of the data segment and
remove a redundant check on CH_PCI_DEVICE_ID_TABLE_DEFINE_BEGIN in
t4_pci_id_tbl.h to guard the contents of the include file.

Pull debugfs patches from Al Viro:
 "debugfs patches, mostly to make it possible for something like tracefs
  to be transparently automounted on given directory in debugfs.

  New primitive in there is debugfs_create_automount(name, parent, func,
  arg), which creates a directory and makes its -&gt;d_automount() return
  func(arg).  Another missing primitive was debugfs_create_file_size() -
  open-coded in quite a few places.  Dave's patch adds it and converts
  the open-code instances to calling it"

This patch fix is to use default firmware configuration files
present in the adapter incase if not available in standard
/lib/firmware/* dir. Additional cleanup is done to reuse flash
related defines from cxgb4 header file.

Please apply over net-next since it depends on previous commit.

We found a subtle issue with FCoE on T4 very late in the game
and decided not to productize FCoE on T4 and therefore there
are no customers that will be impacted by this change. Hence
T4 FCoE support is removed. FCoE supported only on T5 cards.
