scsi: dc395x: remove dc395x_bios_param

dc395x_bios_param was only different from the default when the
CONFIG_SCSI_DC395x_TRMS1040_TRADMAP symbol is true, but that symbol doesn't
exist in the Kconfig system and thus can't be set.

The same effects can be achieved by setting the dma_boundary to
PAGE_SIZE - 1 and the max_segment_size to PAGE_SIZE, so shift those
settings into the drivers.  Note that in many cases the setting might
be bogus, but this keeps the status quo.

We can't just transfer ownership to the CPU and then unmap, as this will
break with swiotlb.

Instead unmap the command and sense buffer a little earlier in the I/O
completion handler and get rid of the pci_dma_sync_sg_for_cpu call
entirely.

When evaluating a SCSI command's result using the field access macros,
check for equality of the fields and not if a specific bit is set.

This is a preparation patch, for reworking the results field in the
SCSI command.
