Protect the megasas_get_ctrl_info() call in megasas_resume() with
instance-&gt;reset_mutex using scoped_guard().

megasas_get_ctrl_info() may release and reacquire instance-&gt;reset_mutex.
Hence, calling this function without holding instance-&gt;reset_mutex is not
safe.

megasas_make_prp_nvme() builds a command's PRP list in cmd-&gt;sg_frame, a
DMA pool buffer of instance-&gt;max_chain_frame_sz bytes, spending one
entry per NVMe page of the transfer plus one per page of the buffer for
the chain pointer. The loop runs until the transfer is described and
never checks the buffer bound.

max_hw_sectors comes straight from the MDTS the firmware reports for the
drive. On drives with a large MDTS the only thing keeping the list
inside the buffer was the block layer default of 1280 KiB, which needs
320 entries, which fit into a 4 KiB frame as that holds 512. But since
commit 9b8b84879d4a ("block: Increase BLK_DEF_MAX_SECTORS_CAP") that
default is 4 MiB, and such a transfer needs 1025 entries, so the list
runs a full page past the end of the frame:

If the page after the frame happens to be mapped, the overrun does not
fault but silently corrupts the neighbouring pool entry, which is
another in-flight command's PRP list.

Cap max_hw_sectors at what the chain frame can describe, less one page
for transfers that do not start on a page boundary and so need one entry
more. This is the megaraid_sas counterpart of commit 04631f55afc5
("scsi: mpt3sas: Limit NVMe request size to 2 MiB"), but derives the
limit from max_chain_frame_sz rather than hardcoding it.

Some but not all SCSI host state changes are protected with the SCSI
host lock. Annotate the SCSI host state with __guarded_by(host_lock) and
protect all SCSI host state changes with the SCSI host lock. This patch
prevents that KCSAN complains about data races when accessing the SCSI
host state.

The megaraid_cmm_register() function has a local copy of mraid_mmadp_t
on the stack that gets copied into the actual structure used at
runtime. When -fsanitize=thread is enabled, this causes the per-function
stack frame to grow beyond the warning limit:

megaraid_mbox.c: In function 'megaraid_cmm_register':
megaraid_mbox.c:3472:1: error: the frame size of 1312 bytes is larger than 1280 bytes [-Werror=frame-larger-than=]

Refactor this by moving the allocation into the caller to save the extra
on-stack copy of the structure.

Add NULL check for scmd_local in the MPI2_FUNCTION_SCSI_IO_REQUEST case
to handle firmware duplicate/stale completions.

When firmware sends a duplicate completion for a command that was
already processed and returned to the pool, the driver accesses NULL
scmd pointer causing a crash.

Timeline of the bug:

 1. Command completes normally, megasas_return_cmd_fusion() called

 2. This sets cmd-&gt;scmd = NULL and clears io_request with memset(..., 0,
    ...)

 3. Firmware sends duplicate/stale completion for same SMID (firmware
    bug)

 4. Driver processes reply descriptor again

 5. Cleared io_request has Function = 0 (MPI2_FUNCTION_SCSI_IO_REQUEST)

 6. Switch statement matches SCSI_IO_REQUEST case by accident

 7. Accesses megasas_priv(NULL scmd)-&gt;status -&gt; crash at offset 0x228

The offset 0x228 = sizeof(struct scsi_cmnd) 0x220 + offsetof(status)
0x8.

This issue was observed on PERC H330 Mini running firmware 25.5.9.0001
after 3+ days of heavy I/O load.

Crash signature:

  BUG: unable to handle kernel NULL pointer dereference at 0x228
  RIP: complete_cmd_fusion+0x428
  Function: megasas_priv(cmd_fusion-&gt;scmd)-&gt;status

Add defensive check to skip processing when scmd_local is NULL. This
handles duplicate completions from firmware and prevents accessing freed
command structures. The check protects all scmd_local uses in both the
SCSI_IO path and the fallthrough LDIO path.

<title>Convert more 'alloc_obj' cases to default GFP_KERNEL arguments</title>

This converts some of the visually simpler cases that have been split
over multiple lines.  I only did the ones that are easy to verify the
resulting diff by having just that final GFP_KERNEL argument on the next
line.

Somebody should probably do a proper coccinelle script for this, but for
me the trivial script actually resulted in an assertion failure in the
middle of the script.  I probably had made it a bit _too_ trivial.

So after fighting that far a while I decided to just do some of the
syntactically simpler cases with variations of the previous 'sed'
scripts.

The more syntactically complex multi-line cases would mostly really want
whitespace cleanup anyway.
