osd: fix signed char versus %02x issue

If char is signed and one of these bytes happen to have a value outside
the ascii range, the corresponding output will consist of "ffffff"
followed by the two hex chars that were actually intended. One way to
fix it would be to change the casts to (u8*) aka
(unsigned char*), but it is much simpler (and generates smaller code)
to use the %ph extension which was created for such short hexdumps.

scsi: remove scsi_driver owner field

The driver core driver structure has grown an owner field and now
requires it to be set for all modular drivers.  Set it up for
all scsi_driver instances and get rid of the now superflous
scsi_driver owner field.

block,scsi: fixup blk_get_request dead queue scenarios

The blk_get_request function may fail in low-memory conditions or during
device removal (even if __GFP_WAIT is set). To distinguish between these
errors, modify the blk_get_request call stack to return the appropriate
ERR_PTR. Verify that all callers check the return status and consider
IS_ERR instead of a simple NULL pointer check.

For consistency, make a similar change to the blk_mq_alloc_request leg
of blk_get_request.  It may fail if the queue is dead, or the caller was
unwilling to wait.

block: add blk_rq_set_block_pc()

With the optimizations around not clearing the full request at alloc
time, we are leaving some of the needed init for REQ_TYPE_BLOCK_PC
up to the user allocating the request.

Add a blk_rq_set_block_pc() that sets the command type to
REQ_TYPE_BLOCK_PC, and properly initializes the members associated
with this type of request. Update callers to use this function instead
of manipulating rq->cmd_type directly.

Includes fixes from Christoph Hellwig  for my half-assed
attempt.

Immutable biovecs are going to require an explicit iterator. To
implement immutable bvecs, a later patch is going to add a bi_bvec_done
member to this struct; for now, this patch effectively just renames
things.

SCSI: OSD: convert class code to use dev_groups

The dev_attrs field of struct class is going away soon, dev_groups
should be used instead.  This converts the scsi osd class code to use
the correct field.
