The same effects can be achieved by setting the dma_boundary to
PAGE_SIZE - 1 and the max_segment_size to PAGE_SIZE, so shift those
settings into the drivers.  Note that in many cases the setting might
be bogus, but this keeps the status quo.

When the kernel is running in secure boot mode, we lock down the kernel to
prevent userspace from modifying the running kernel image.  Whilst this
includes prohibiting access to things like /dev/mem, it must also prevent
access by means of configuring driver modules in such a way as to cause a
device to access or modify the kernel image.

To this end, annotate module_param* statements that refer to hardware
configuration and indicate for future reference what type of parameter they
specify.  The parameter parser in the core sees this information and can
skip such parameters with an error message if the kernel is locked down.
The module initialisation then runs as normal, but just sees whatever the
default values for those parameters is.

Note that we do still need to do the module initialisation because some
drivers have viable defaults set in case parameters aren't specified and
some drivers support automatic configuration (e.g. PNP or PCI) in addition
to manually coded parameters.

This patch annotates drivers in drivers/scsi/.

'0' is now used as the default cmd_per_lun value,

The qlogicfas scsi driver does not use DMA, and the call to free_dma()
in its exit function seems to have been copied incorrectly from
another driver but never caused trouble.

One case where it gets in the way is randconfig builds on ARM,
which depending on the configuration does not provide a free_dma()
function, causing this build error:

CONFIG_HOTPLUG is going away as an option.  As a result, the __dev*
markings need to be removed.

This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.

Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

With the sg table code, every SCSI driver is now either chain capable
or broken (or has sg_tablesize set so chaining is never activated), so
there's no need to have a check in the host template.

Also tidy up the code by moving the scatterlist size defines into the
SCSI includes and permit the last entry of the scatterlist pools not
to be a power of two.

This option is true if a low-level driver can support sg
chaining. This will be removed eventually when all the drivers are
converted to support sg chaining. q->max_phys_segments is set to
SCSI_MAX_SG_SEGMENTS if false.

We were releasing the IRQ before removing the host, so commands could
still be coming in which would never be seen by the interrupt handler.
Just remove the host before releasing the IRQ to close this race.
