<title>scsi: Rename .slave_alloc() and .slave_destroy()</title>

Rename .slave_alloc() into .sdev_init() and .slave_destroy() into
.sdev_destroy(). The new names make it clear that these are actions on
SCSI devices. Make this change in the SCSI core, SCSI drivers and also
in the ATA drivers. No functionality has been changed.

This patch has been created as follows:
* Change the text "slave_alloc" into "sdev_init" in all source files
  except those in drivers/net/ and Documentation/.
* Change the text "slave_destroy" into "sdev_destroy" in all source
  files except those in drivers/net/ and Documentation/.
* Rename lpfc_no_slave() into lpfc_no_sdev().
* Manually adjust whitespace where necessary to restore vertical
  alignment (dc395x driver and include/linux/libata.h).

Instead of passing separate names and callbacks arrays

This adds polling support to virtio-scsi. It's based on and works similar
to virtblk support where we add a module param to specify the number of
poll queues then subtract to calculate the IO queues.

When using 8 poll queues and a vhost worker per queue we see 4K IOPs
with fio:

fio --filename=/dev/sda --direct=1 --rw=randread --bs=4k \
--ioengine=io_uring --hipri --iodepth=128  --numjobs=$NUM_JOBS

increase like:

jobs	base	poll
1	207K	296K
2	392K	552K
3	581K	860K
4	765K	1235K
5	936K	1598K

Most callers of scsi_rescan_device() have the scsi_device pointer readily
available. Pass a struct scsi_device pointer to scsi_rescan_device()
instead of a struct device pointer. This change prevents that a pointer to
another struct device would be passed accidentally to scsi_rescan_device().

Remove the scsi_rescan_device() declaration from the scsi_priv.h header
file since it duplicates the declaration in &lt;scsi/scsi_host.h&gt;.
