Fix null-pointer dereference during probe if the interface-status
completion handler is called before the individual ports have been set
up.

Fix handling of TTY error flags, which are not bitmasks and must
specifically not be ORed together as this prevents the line discipline
from recognising them.

Also insert null characters when reporting overrun errors as these are
not associated with the received character.

Make sure to fail properly if the device is not accepted during attach
in order to avoid null-pointer derefs (of missing interface private
data) at disconnect or release.

Pull USB patches from Greg Kroah-Hartman:
 "Here's the big USB pull request for 3.10-rc1.

  Lots of USB patches here, the majority being USB gadget changes and
  USB-serial driver cleanups, the rest being ARM build fixes / cleanups,
  and individual driver updates.  We also finally got some chipidea
  fixes, which have been delayed for a number of kernel releases, as the
  maintainer has now reappeared.

"len" comes from the USB transfer and it's probably correct.  The thing
is that we already have similar checks like:

        if (data[i] &gt;= serial-&gt;num_ports) {

So adding a sanity test here matches the rest of the code and is a good
idea.
