Fix NULL-pointer dereference in open() should the device lack the
expected endpoints:

Unable to handle kernel NULL pointer dereference at virtual address 00000030
...
PC is at oti6858_open+0x30/0x1d0 [oti6858]

Note that a missing interrupt-in endpoint would have caused open() to
fail.

None of these files are actually using any __init type directives
and hence don't need to include &lt;linux/init.h&gt;.  Most are just a
left over from __devinit and __cpuinit removal, or simply due to
code getting copied from one driver to the next.

Make sure set_termios is not called with uninitialised data at open. The
old termios struct is currently not used, but pass NULL instead to avoid
future problems (e.g. stack data leak).

Use tty-port modem-status-change wait queue on which processes are woken
up at hangup and disconnect.

Currently a process waiting on modem-status changes will not be woken on
device disconnect.
