Export the number of successful and failed hugetlb page
allocations via the virtio balloon driver. These 2 counts
come directly from the vm_events HTLB_BUDDY_PGALLOC and
HTLB_BUDDY_PGALLOC_FAIL.

headers: untangle kmemleak.h from mm.h

Currently  #includes  for no obvious
reason.  It looks like it's only a convenience, so remove kmemleak.h
from slab.h and add  to any users of kmemleak_* that
don't already #include it.  Also remove  from source
files that do not use it.

This is tested on i386 allmodconfig and x86_64 allmodconfig.  It would
be good to run it through the 0day bot for other $ARCHes.  I have
neither the horsepower nor the storage space for the other $ARCHes.

Update: This patch has been extensively build-tested by both the 0day
bot & kisskb/ozlabs build farms.  Both of them reported 2 build failures
for which patches are included here (in v2).

[ slab.h is the second most used header file after module.h; kernel.h is
  right there with slab.h. There could be some minor error in the
  counting due to some #includes having comments after them and I didn't
  combine all of those. ]

virtio: split device_register into device_initialize and device_add

In order to make caller do a simple cleanup, we split device_register
into device_initialize and device_add. device_initialize always succeeds,
so the caller can always use put_device when register_virtio_device faild.

virtio: make VIRTIO a menuconfig to ease disabling it all

No need to get into the submenu to disable all VIRTIO-related
config entries.

This makes it easier to disable all VIRTIO config options
without entering the submenu. It will also enable one
to see that en/dis-abled state from the outside menu.

This is only intended to change menuconfig UI, not change
the config dependencies.

virtio: virtio_mmio: make of_device_ids const.

of_device_ids are not supposed to change at runtime. All functions
working with of_device_ids provided by  work with const
of_device_ids. So mark the non-const structs as const.

virtio_balloon: include disk/file caches memory statistics

Add a new field VIRTIO_BALLOON_S_CACHES to virtio_balloon memory
statistics protocol. The value represents all disk/file caches.

In this case it corresponds to the sum of values
Buffers+Cached+SwapCached from /proc/meminfo.
