Remove the SLAB specific function slab_should_failslab(), by moving the
check against fault-injection for the bootstrap slab, into the shared
function should_failslab() (used by both SLAB and SLUB).

This is a step towards sharing alloc_hook's between SLUB and SLAB.

This bootstrap slab "kmem_cache" is used for allocating struct
kmem_cache objects to the allocator itself.

Its a bit odd that debugfs_create_bool() takes 'u32 *' as an argument,
when all it needs is a boolean pointer.

It would be better to update this API to make it accept 'bool *'
instead, as that will make it more consistent and often more convenient.
Over that bool takes just a byte.

That required updates to all user sites as well, in the same commit
updating the API. regmap core was also using
debugfs_{read|write}_file_bool(), directly and variable types were
updated for that to be bool as well.

Current debug levels are not optimal.  Especially if one want to provoke
big numbers of faults(broken device simulator) then any verbose level will
produce giant numbers of identical logging messages.  Let's add ratelimit
parameter for that purpose.

init_fault_attr_dentries() is used to export fault_attr via debugfs.
But it can only export it in debugfs root directory.

Per Forlin is working on mmc_fail_request which adds support to inject
data errors after a completed host transfer in MMC subsystem.

The fault_attr for mmc_fail_request should be defined per mmc host and
export it in debugfs directory per mmc host like
/sys/kernel/debug/mmc0/mmc_fail_request.

init_fault_attr_dentries() doesn't help for mmc_fail_request.  So this
introduces fault_create_debugfs_attr() which is able to create a
directory in the arbitrary directory and replace
init_fault_attr_dentries().

This patch allow to inject faults only for specific slabs.
In order to preserve default behavior cache filter is off by
default (all caches are faulty).

One may define specific set of slabs like this:
# mark skbuff_head_cache as faulty
echo 1 &gt; /sys/kernel/slab/skbuff_head_cache/failslab
# Turn on cache filter (off by default)
echo 1 &gt; /sys/kernel/debug/failslab/cache-filter
# Turn on fault injection
echo 1 &gt; /sys/kernel/debug/failslab/times
echo 1 &gt; /sys/kernel/debug/failslab/probability
