When I tried to clean up the driver a bit, Arnd noted:
> According to thinkwiki.de, the 3780i modem was only used in a
> couple of Thinkpad models that are now over 25 years old, using
> Pentium II processors, and they all have a physical RS232 port
> that can be used to connect an external modem instead.
>
> Maybe we can just retire this driver?

So instead of the clean up, drop the driver altogether.

Currently, It is allowed to register miscdevice with minor > 255
which is defined by macro MISC_DYNAMIC_MINOR, and cause:

- Chaos regarding division and management of minor codes.
- Registering failure if the minor was allocated to other dynamic request.

Fortunately, in-kernel users have not had such usage yet.
Fix by refusing to register miscdevice whose minor > 255.

Also bring in a very simple minor code space division and management:

< 255 : Fixed minor code
== 255 : Indicator to request dynamic minor code
> 255 : Dynamic minor code requested, 1048320 minor codes totally
And all fixed minors allocated should be registered in 'linux/miscdevice.h'

When one is too lazy to use an LSP to conduct look-ups on struct
definitions, one might use the ever useful `struct <name> {` search
string. However this doesn't work with `struct miscdevice {` because
of a stray double space. Assuming that this wasn't intentional, let's
simply remove it.

There are two EFI RTC drivers, the original drivers/char/efirtc.c
driver and the more modern drivers/rtc/rtc-efi.c.

Both implement the same interface, but the new one does so
in a more portable way.

Move everything over to that one and remove the old one.
