mm/mremap: simplify vma expansion again

This effectively reverts d014cd7c1c35 ("mm, mremap: fix mremap() expanding
for vma's with vm_ops->close()").  After the recent changes, vma_merge()
is able to handle the expansion properly even when the vma being expanded
has a vm_ops->close operation, so we don't need to special case it
anymore.

x86/mm/pat: clear VM_PAT if copy_p4d_range failed

Syzbot reports a warning in untrack_pfn().  Digging into the root we found
that this is due to memory allocation failure in pmd_alloc_one.  And this
failure is produced due to failslab.

In copy_page_range(), memory alloaction for pmd failed.  During the error
handling process in copy_page_range(), mmput() is called to remove all
vmas.  While untrack_pfn this empty pfn, warning happens.

Here's a simplified flow:

      copy_p4d_range
        copy_pud_range
          copy_pmd_range
            pmd_alloc
              __pmd_alloc
                pmd_alloc_one
                  page = alloc_pages(gfp, 0);
                    if (!page)

          unmap_vmas
            unmap_single_vma
              untrack_pfn
                follow_phys
                  WARN_ON_ONCE(1);

Since this vma is not generate successfully, we can clear flag VM_PAT.  In
this case, untrack_pfn() will not be called while cleaning this vma.

Function untrack_pfn_moved() has also been renamed to fit the new logic.

Use the abstracted locking and maple tree operations.  Since __split_vma()
is the only user of the __vma_adjust() function to use the insert
argument, drop that argument.  Remove the NULL passed through from
fs/exec's shift_arg_pages() and mremap() at the same time.

mm: change munmap splitting order and move_vma()

Splitting can be more efficient when the order is not of concern.  Change
do_vmi_align_munmap() to reduce walking of the tree during split
operations.

move_vma() must also be altered to remove the dependency of keeping the
original VMA as the active part of the split.  Transition to using vma
iterator to look up the prev and/or next vma after munmap.

mm: add vma iterator to vma_adjust() arguments

Change the vma_adjust() function definition to accept the vma iterator and
pass it through to __vma_adjust().

Update fs/exec to use the new vma_adjust() function parameters.

Update mm/mremap to use the new vma_adjust() function parameters.

Revert the __split_vma() calls back from __vma_adjust() to vma_adjust()
and pass through the vma iterator.
