ARM: tegra: iommu: Clean up checkpatch and CodingStyle errors
- Fixed checkpatch.pl --strict errors.
- Inserted one space around binary operators
From Documentation/CodingStyle "3.1: Spaces".
- Removed a file path line in the head of file.
- Updated Copyright year
- Removed duplicated header inclusions
