2013-03-14powerpc: Apply early paca fixups to boot_paca and the boot cpu's pacaMichael Ellerman

In commit 466921c we added a hack to set the paca data_offset to zero so that per-cpu accesses would work on the boot cpu prior to per-cpu areas being setup. This fixed a problem with lockdep touching per-cpu areas very early in boot. However if we combine CONFIG_LOCK_STAT=y with any of the PPC_EARLY_DEBUG options, we can hit the same problem in udbg_early_init(). To avoid that we need to set the data_offset of the boot_paca also. So factor out the fixup logic and call it for both the boot_paca, and "the paca of the boot cpu". 2013-03-14ARM: kirkwood: of_serial: fix clock gating by removing clock-frequencyJason Cooper
commit 93fff4ce19f9978cc1c59db42760717477939249 upstream. When DT support for kirkwood was first introduced, there was no clock infrastructure. As a result, we had to manually pass the clock-frequency to the driver from the device node. Unfortunately, on kirkwood, with minimal config or all module configs, clock-frequency breaks booting because of_serial doesn't consume the gate_clk when clock-frequency is defined. The end result on kirkwood is that runit gets gated, and then the boot fails when the kernel tries to write to the serial port. Fix the issue by removing the clock-frequency parameter from all kirkwood dts files. Booted on dreamplug without earlyprintk and successfully logged in via ttyS0. 2013-03-14tile: work around bug in the generic sys_llseekChris Metcalf
commit 5a114b98661e3aaa0ac085eb931584dce3b0ef9b upstream. sys_llseek should specify the high and low 32-bit seek values as "unsigned int" but instead it specifies "unsigned long". Since compat syscall arguments are always sign-extended on tile, this means that a seek value of 0xffffffff will be incorrectly interpreted as a value of -1ULL. To avoid the risk of breaking binary compatibility on architectures that already use sys_llseek this way, we follow the same path as MIPS and provide a wrapper override. 2013-03-14ARM: 7659/1: mm: make mm->context.id an atomic64_t variableWill Deacon
commit 8a4e3a9ead7e37ce1505602b564c15da09ac039f upstream. mm->context.id is updated under asid_lock when a new ASID is allocated to an mm_struct. However, it is also read without the lock when a task is being scheduled and checking whether or not the current ASID generation is up-to-date. If two threads of the same process are being scheduled in parallel and the bottom bits of the generation in their mm->context.id match the current generation (that is, the mm_struct has not been used for ~2^24 rollovers) then the non-atomic, lockless access to mm->context.id may yield the incorrect ASID. This patch fixes this issue by making mm->context.id and atomic64_t, ensuring that the generation is always read consistently. For code that only requires access to the ASID bits (e.g. TLB flushing by mm), then the value is accessed directly, which GCC converts to an ldrb. 2013-03-14ARM: 7658/1: mm: fix race updating mm->context.id on ASID rolloverWill Deacon

If a thread triggers an ASID rollover, other threads of the same process must be made to wait until the mm->context.id for the shared mm_struct has been updated to new generation and associated book-keeping (e.g. TLB invalidation) has ben performed. However, there is a *tiny* window where both mm->context.id and the relevant active_asids entry are updated to the new generation, but the TLB flush has not been performed, which could allow another thread to return to userspace with a dirty TLB, potentially leading to data corruption. In reality this will never occur because one CPU would need to perform a context-switch in the time it takes another to do a couple of atomic test/set operations but we should plug the race anyway. This patch moves the active_asids update until after the potential TLB flush on context-switch. As such with these two new commits: - 08261d87f7d1b6253ab3223756625a5c74532293 PCI/MSI: Enable multiple MSIs with pci_enable_msi_block_auto() - 5ca72c4f7c412c2002363218901eba5516c476b1 AHCI: Support multiple MSIs we would call the PHYSDEVOP_map_pirq 'nvec' times with the same contents of the PCI device. Sander discovered that we would get the same PIRQ value 'nvec' times and return said values to the caller. Following commit 26ffd0d4 (ARM: mm: introduce present, faulting entries for PAGE_NONE), if a page has been mapped as PROT_NONE, the L_PTE_VALID bit is cleared by the set_pte_ext() code. With LPAE the software and hardware pte share the same location and subsequent modifications of pte range (change_protection()) will leave the L_PTE_VALID bit cleared. This patch adds the L_PTE_VALID bit to the newprot mask in pte_modify(). 2013-03-14ARM: 7653/2: do not scale loops_per_jiffy when using a constant delay clockNicolas Pitre

When udelay() is implemented using an architected timer, it is wrong to scale loops_per_jiffy when changing the CPU clock frequency since the timer clock remains constant. The lpj should probably become an implementation detail relevant to the CPU loop based delay routine only and more confined to it. In the mean time this is the minimal fix needed to have expected delays with the timer based implementation when cpufreq is also in use. Martin Storsjö reports that the sequence: ee312ac1 vsub.f32 s4, s3, s2 ee702ac0 vsub.f32 s5, s1, s0 e59f0028 ldr r0, [pc, #40] ee111a90 vmov r1, s3 on Raspberry Pi (implementor 41 architecture 1 part 20 variant b rev 5) where s3 is a denormal and s2 is zero results in incorrect behaviour - the instruction "vsub.f32 s5, s1, s0" is not executed: VFP: bounce: trigger ee111a90 fpexc d0000780 VFP: emulate: INST=0xee312ac1 SCR=0x00000000 ... As we can see, the instruction triggering the exception is the "vmov" instruction, and we emulate the "vsub.f32 s4, s3, s2" but fail to properly take account of the FPEXC_FP2V flag in FPEXC. This is because the test for the second instruction register being valid is bogus, and will always skip emulation of the second instruction. When initrd file didn't put at the same place with stub kernel, we need give the file path of initrd, but need use backslash to separate directory and file. It's not friendly to unix/linux user, and not so intuitive for bootloader forward paramters to efi stub kernel by chainloading. This patch add support to handle_ramdisks for allow slash in file path of initrd, it convert slash to backlash when parsing path. In additional, this patch also separates print code of efi_char16_t from efi_printk, and print out the path/filename of initrd when failed to open initrd file. It's good for debug and discover typo. On non-BIOS platforms it is possible that the BIOS data area contains garbage instead of being zeroed or something equivalent (firmware people: we are talking of 1.5K here, so please do the sane thing.) We need on the order of 20-30K of low memory in order to boot, which may grow up to < 64K in the future. We probably want to avoid the lowest of the low memory. At the same time, it seems extremely unlikely that a legitimate EBDA would ever reach down to the 128K (which would require it to be over half a megabyte in size.) Thus, pick 128K as the cutoff for "this is insane, ignore." We may still end up reserving a bunch of extra memory on the low megabyte, but that is not really a major issue these days. In the worst case we lose 512K of RAM. This code really should be merged with trim_bios_range() in arch/x86/kernel/setup.c, but that is a bigger patch for a later merge window. 2013-03-04x86, efi: Make "noefi" really disable EFI runtime serivcesMatt Fleming
commit fb834c7acc5e140cf4f9e86da93a66de8c0514da upstream. commit 1de63d60cd5b ("efi: Clear EFI_RUNTIME_SERVICES rather than EFI_BOOT by "noefi" boot parameter") attempted to make "noefi" true to its documentation and disable EFI runtime services to prevent the bricking bug described in commit e0094244e41c ("samsung-laptop: Disable on EFI hardware"). However, it's not possible to clear EFI_RUNTIME_SERVICES from an early param function because EFI_RUNTIME_SERVICES is set in efi_init() *after* parse_early_param(). This resulted in "noefi" effectively becoming a no-op and no longer providing users with a way to disable EFI, which is bad for those users that have buggy machines. 2013-03-04x86/apic: Fix parsing of the 'lapic' cmdline optionMathias Krause
commit 27cf929845b10043f2257693c7d179a9e0b1980e upstream. Including " lapic " in the kernel cmdline on an x86-64 kernel makes it panic while parsing early params -- e.g. with no user visible output. Fix this bug by ensuring arg is non-NULL before passing it to strncmp(). 2013-02-28powerpc/eeh: Fix crash when adding a device in a slot with DDWThadeu Lima de Souza Cascardo
commit 6a040ce72598159a74969a2d01ab0ba5ee6536b3 upstream. The DDW code uses a eeh_dev struct from the pci_dev. However, this is not set until eeh_add_device_late is called. Since pci_bus_add_devices is called before eeh_add_device_late, the PCI devices are added to the bus, making drivers' probe hooks to be called. These will call set_dma_mask, which will call the DDW code, which will require the eeh_dev struct from pci_dev. This would result in a crash, due to a NULL dereference. Calling eeh_add_device_late after pci_bus_add_devices would make the system BUG, because device files shouldn't be added to devices there were not added to the system. So, a new function is needed to add such files only after pci_bus_add_devices have been called. But, output in example case are not. So, change updating sequence in order to correct this problem. 2013-02-28ARM: 7635/1: versatile: fix the PCI IRQ regressionLinus Walleij
commit e3e92a7be6936dff1de80e66b0b683d54e9e02d8 upstream. The PCI IRQs were regressing due to two things: - The PCI glue layer was using an hard-coded IRQ 27 offset. This caused the immediate regression. - The SIC IRQ mask was inverted (i.e. a bit was indeed set to one for each valid IRQ on the SIC, but accidentally inverted in the init call). This has been around forever, but we have been saved by some other forgiving code that would reserve IRQ descriptors in this range, as the versatile is non-sparse. When the IRQs were bumped up 32 steps so as to avoid using IRQ zero and avoid touching the 16 legacy IRQs, things broke. Introduce an explicit valid mask for the IRQs that are active on the PIC/SIC, and pass that. Use the BIT() macro from <linux/bitops.h> to make sure we hit the right bits, readily defined in <mach/platform.h>. So the above actually assembles into: or REG, -4096, REG completely corrupting the final PTE value. Set it with a: sethi %hi(_PAGE_EXEC_4U), TMP or REG, TMP, REG sequence instead. [ Upstream commit 0fbebed682ff2788dee58e8d7f7dda46e33aa10b ] If our first THP installation for an MM is via the set_pmd_at() done during khugepaged's collapsing we'll end up in tsb_grow() trying to do a GFP_KERNEL allocation with several locks held. Simply using GFP_ATOMIC in this situation is not the best option because we really can't have this fail, so we'd really like to keep this an order 0 GFP_KERNEL allocation if possible. Also, doing the TSB allocation from khugepaged is a really bad idea because we'll allocate it potentially from the wrong NUMA node in that context. So what we do is defer the hugepage TSB allocation until the first TLB miss we take on a hugepage. This is slightly tricky because we have to handle two unusual cases: 1) Taking the first hugepage TLB miss in the window trap handler. We'll call the winfix_trampoline when that is detected. 2) An initial TSB allocation via TLB miss races with a hugetlb fault on another cpu running the same MM. Currently we might save stale versions: The sie state descriptor doesnt have fields for guest ACRS,FPRS, those registers are simply stored in the host registers. The host program must copy these away if needed. We do that in vcpu_put/load. If we now do a store status in KVM code between vcpu_put/load, the saved values are not up-to-date. Lets collect the ACRS/FPRS before saving them. This also fixes some strange problems with hotplug and virtio-ccw, since the low level machine check handler (on hotplug a machine check will happen) will revalidate all registers with the content of the save area. 2013-02-28xen: Send spinlock IPI to all waitersStefan Bader
commit 76eaca031f0af2bb303e405986f637811956a422 upstream. There is a loophole between Xen's current implementation of pv-spinlocks and the scheduler. This was triggerable through a testcase until v3.6 changed the TLB flushing code. The problem potentially is still there just not observable in the same way. What could happen was (is): 1. CPU n tries to schedule task x away and goes into a slow wait for the runq lock of CPU n-# (must be one with a lower number). 2. CPU n-#, while processing softirqs, tries to balance domains and goes into a slow wait for its own runq lock (for updating some records). Since this is a spin_lock_irqsave in softirq context, interrupts will be re-enabled for the duration of the poll_irq hypercall used by Xen. 3. Before the runq lock of CPU n-# is unlocked, CPU n-1 receives an interrupt (e.g. endio) and when processing the interrupt, tries to wake up task x. But that is in schedule and still on_cpu, so try_to_wake_up goes into a tight loop. 4. The runq lock of CPU n-# gets unlocked, but the message only gets sent to the first waiter, which is CPU n-# and that is busily stuck. 5. CPU n-# never returns from the nested interruption to take and release the lock because the scheduler uses a busy wait. And CPU n never finishes the task migration because the unlock notification only went to CPU n-#. To avoid this and since the unlocking code has no real sense of which waiter is best suited to grab the lock, just send the IPI to all of them. This causes the waiters to return from the hyper- call (those not interrupted at least) and do active spinlocking. XenServer 6 returns the signature which is checked in ms_hyperv_platform(), but it does not offer all features. Currently the clocksource is enabled unconditionally in ms_hyperv_init_platform(), and the result is a hanging guest. Hyper-V spec Bit 1 indicates the availability of Partition Reference Counter. Register the clocksource only if this bit is set. The guest in question prints this in dmesg: [ 0.000000] Hypervisor detected: Microsoft HyperV [ 0.000000] HyperV: features 0x70, hints 0x0 This bug can be reproduced easily be setting 'viridian=1' in a HVM domU .cfg file. A workaround without this patch is to boot the HVM guest with 'clocksource=jiffies'. commit f03574f2d5b2d6229dcdf2d322848065f72953c7 upstream. This code was an optimization for 32-bit NUMA systems. It has probably been the cause of a number of subtle bugs over the years, although the conditions to excite them would have been hard to trigger. Essentially, we remap part of the kernel linear mapping area, and then sometimes part of that area gets freed back in to the bootmem allocator. If those pages get used by kernel data structures (say mem_map[] or a dentry), there's no big deal. But, if anyone ever tried to use the linear mapping for these pages _and_ cared about their physical address, bad things happen. For instance, say you passed __GFP_ZERO to the page allocator and then happened to get handed one of these pages, it zero the remapped page, but it would make a pte to the _old_ page. There are probably a hundred other ways that it could screw with things. We don't need to hang on to performance optimizations for these old boxes any more. All my 32-bit NUMA systems are long dead and buried, and I probably had access to more than most people. This reverts commit 9d02b43dee0d7fb18dfb13a00915550b1a3daa9f. We are doing this b/c on 32-bit PVonHVM with older hypervisors (Xen 4.1) it ends up bothing up the start_info. This is bad b/c we use it for the time keeping, and the timekeeping code loops forever - as the version field never changes. Olaf says to revert it, so lets do that. " Jan took a look at the preliminary patch and came up a fix that solves this problem: "This code gets called after all registers other than those handled by IRET got already restored, hence a null selector in %ds or a non-null one that got loaded from a code or read-only data descriptor would cause a kernel mode fault (with the potential of crashing the kernel as a whole, if panic_on_oops is set)." The way to fix this is to realize that the we can only relay on the registers that IRET restores. The two that are guaranteed are the %cs and %ss as they are always fixed GDT selectors. Also they are inaccessible from user mode - so they cannot be altered. This is the approach taken in this patch. Another alternative option suggested by Jan would be to relay on the subtle realization that using the %ebp or %esp relative references uses the %ss segment. In which case we could switch from using %eax to %ebp and would not need the %ss over-rides. That would also require one extra instruction to compensate for the one place where the register is used as scaled index. However Andrew pointed out that is too subtle and if further work was to be done in this code-path it could escape folks attention and lead to accidents.
