Since ipoib_cm_tx_start function and ipoib_cm_tx_reap function
belong to different work queues, they can run in parallel.
In this case if ipoib_cm_tx_reap calls list_del and release the
lock, ipoib_cm_tx_start may acquire it and call list_del_init
on the already deleted object.
Changing list_del to list_del_init in ipoib_cm_tx_reap fixes the problem.

In order to resolve a situation where a few process delete
the same list element in sequence and cause panic, list_del
is replaced with list_del_init. In this case if the first
process that calls list_del releases the lock before acquiring
it again, other processes who can acquire the lock will call
list_del_init.

The ipoib_vlan_add function calls rtnl_unlock after free_netdev,
rtnl_unlock not only releases the lock, but also calls netdev_run_todo.
The latter function browses the net_todo_list array and completes the
unregistration of all its net_device instances. If we call free_netdev
before rtnl_unlock, then netdev_run_todo call over the freed device causes
panic.
To fix, move rtnl_unlock call before free_netdev call.

This patch fixes Deadlock while executing ipoib_vlan_delete.
The function takes the vlan_rwsem semaphore and calls
unregister_netdevice. The later function calls
ipoib_mcast_stop_thread that cause workqueue flush.
When the queue has one of the ipoib_ib_dev_flush_xxx events,
a deadlock occur because these events also tries to catch the
same vlan_rwsem semaphore.
To fix, unregister_netdevice should be called after releasing
the semaphore.

This patch fixes a NULL pointer dereference in isert_login_recv_done()
of isert_conn->cm_id due to isert_cma_handler() -> isert_connect_error()
resetting isert_conn->cm_id = NULL during a failed login attempt.
As per Sagi, we will always see the completion of all recv wrs posted
on the qp (given that we assigned a ->done handler), this is a FLUSH
error completion, we just don't get to verify that because we deref
NULL before.
The issue here, was the assumption that dereferencing the connection
cm_id is always safe, which is not true since:

The port number is only valid if IB_QP_PORT is set in the mask.
So only check port number if it is valid to prevent modify_qp from
failing due to an invalid port number.

The ib_uverbs_create_ah() ind ib_uverbs_modify_qp() calls receive
the port number from user input as part of its attributes and assumes
it is valid. Down on the stack, that parameter is used to access kernel
data structures. If the value is invalid, the kernel accesses memory
it should not. To prevent this, verify the port number before using it.

The handling of IB_RDMA_WRITE_ONLY_WITH_IMMEDIATE will leak a memory
reference when a buffer cannot be allocated for returning the immediate
data.
The issue is that the rkey validation has already occurred and the RNR
nak fails to release the reference that was fruitlessly gotten. The
the peer will send the identical single packet request when its RNR
timer pops.
The fix is to release the held reference prior to the rnr nak exit.
This is the only sequence the requires both rkey validation and the
buffer allocation on the same packet.

The infiniband address handle can be triggered to resolve an ipv6
address in response to MAD packets, regardless of the ipv6
module being disabled via the kernel command line argument.
That will cause a call into the ipv6 routing code, which is not
initialized, and a conseguent oops.
This commit addresses the above issue replacing the direct lookup
call with an indirect one via the ipv6 stub, which is properly
initialized according to the ipv6 status (e.g. if ipv6 is
disabled, the routing lookup fails gracefully)

A warning message during SRIOV multicast cleanup should have actually been
a debug level message. The condition generating the warning does no harm
and can fill the message log.
In some cases, during testing, some tests were so intense as to swamp the
message log with these warning messages, causing a stall in the console
message log output task. This stall caused an NMI to be sent to all CPUs
(so that they all dumped their stacks into the message log).
Aside from the message flood causing an NMI, the tests all passed.
Once the message flood which caused the NMI is removed (by reducing the
warning message to debug level), the NMI no longer occurs.

When udev renames the netdev devices, ipoib debugfs entries does not
get renamed. As a result, if subsequent probe of ipoib device reuse the
name then creating a debugfs entry for the new device would fail.
Also, moved ipoib_create_debug_files and ipoib_delete_debug_files as part
of ipoib event handling in order to avoid any race condition between these.

The kernel commit cited below restructured ib device management
so that the device kobject is initialized in ib_alloc_device.
As part of the restructuring, the kobject is now initialized in
procedure ib_alloc_device, and is later added to the device hierarchy
in the ib_register_device call stack, in procedure
ib_device_register_sysfs (which calls device_add).
However, in the ib_device_register_sysfs error flow, if an error
occurs following the call to device_add, the cleanup procedure
device_unregister is called. This call results in the device object
being deleted -- which results in various use-after-free crashes.
The correct cleanup call is device_del -- which undoes device_add
without deleting the device object.
The device object will then (correctly) be deleted in the
ib_register_device caller's error cleanup flow, when the caller invokes
ib_dealloc_device.

We get this build warning on arm64

This is fixed upstream in commit 898fa52b4ac3 ("IB/qib: Remove qpn, qp tables and
related variables from qib"), which does a lot of other things as well.
Instead, I just backport the rename of the local BITS_PER_PAGE definition to
RVT_BITS_PER_PAGE.
The driver first showed up in linux-2.6.35, and the fixup should still apply
to that. The upstream fix went into v4.6, so we could apply this workaround
to both 3.18 and 4.4.

Avoid that srp_process_rsp() overwrites the status information
in ch if the SRP target response timed out and processing of
another task management function has already started. Avoid that
issuing multiple task management functions concurrently triggers

When sending packet to destination that was not resolved yet
via path query, the driver keeps the skb and tries to re-send it
again when the path is resolved.
But when re-sending via dev_queue_xmit the kernel doesn't call
to dev_hard_header, so IPoIB needs to keep 20 bytes in the skb
and to put the destination address inside them.
In that way the dev_start_xmit will have the correct destination,
and the driver won't take the destination from the skb->data, while
nothing exists there, which causes to packet be be dropped.
The test flow is:
1. Run the SM on remote node,
2. Restart the driver.
4. Ping some destination,
3. Observe that first ICMP request will be dropped.

When calling set_mode from sys/fs, the call flow locks the sys/fs lock
first and then tries to lock rtnl_lock (when calling ipoib_set_mod).
On the other hand, the rmmod call flow takes the rtnl_lock first
(when calling unregister_netdev) and then tries to take the sys/fs
lock. Deadlock a->b, b->a.
The problem starts when ipoib_set_mod frees it's rtnl_lck and tries
to get it after that.

After the commit 9207f9d45b0a ("net: preserve IP control block
during GSO segmentation"), the GSO CB and the IPoIB CB conflict.
That destroy the IPoIB address information cached there,
causing a severe performance regression, as better described here:

This change moves the data cached by the IPoIB driver from the
skb control lock into the IPoIB hard header, as done before
the commit 936d7de3d736 ("IPoIB: Stop lying about hard_header_len
and use skb->cb to stash LL addresses").
In order to avoid GRO issue, on packet reception, the IPoIB driver
stash into the skb a dummy pseudo header, so that the received
packets have actually a hard header matching the declared length.
To avoid changing the connected mode maximum mtu, the allocated
head buffer size is increased by the pseudo header length.
After this commit, IPoIB performances are back to pre-regression
value.

If IPV6 has not been enabled in the underlying kernel, we must avoid
calling IPV6 procedures in rdma_cm.ko.
This requires using "IS_ENABLED(CONFIG_IPV6)" in "if" statements
surrounding any code which calls external IPV6 procedures.
In the instance fixed here, procedure cma_bind_addr() called
ipv6_addr_type() -- which resulted in calling external procedure
__ipv6_addr_type().

According to the firmware spec, FLOW_STEERING_IB_UC_QP_RANGE command is
supported only if dmfs_ipoib bit is set.
If it isn't set we want to ensure allocating NET_IF QPs fail. We do so
by filling out the allocation bitmap. By thus, the NET_IF QPs allocating
function won't find any free QP and will fail.

For non-special QPs, the port value becomes non-zero only at the
RESET-to-INIT transition. If the QP has not undergone that transition,
its port number value is still zero.
If such a QP is destroyed before being moved out of the RESET state,
subtracting one from the qp port number results in a negative value.
Using that negative value as an index into the qp1_proxy array
results in an out-of-bounds array reference.
Fix this by testing that the QP type is one that uses qp1_proxy before
using the port number. For special QPs of all types, the port number is
specified at QP creation time.

The array ib_mad_mgmt_class_table.method_table has MAX_MGMT_CLASS
(80) elements. Hence compare the array index with that value instead
of with IB_MGMT_MAX_METHODS (128). This patch avoids that Coverity
reports the following:
Overrunning array class->method_table of 80 8-byte elements at element index 127 (byte offset 1016) using index convert_mgmt_class(mad_hdr->mgmt_class) (which evaluates to 127).

When there is a CM id object that has port assigned to it, it means that
the cm-id asked for the specific port that it should go by it, but if
that port was removed (hot-unplug event) the cm-id was not updated.
In order to fix that the port keeps a list of all the cm-id's that are
planning to go by it, whenever the port is removed it marks all of them
as invalid.
This commit fixes a kernel panic which happens when running traffic between
guests and we force reboot a guest mid traffic, it triggers a kernel panic:

When creating kernel CQs use 128B CQE stride if the
cache line size is 128B, 64B otherwise. This prevents
multiple CQEs from residing in a 128B cache line,
which can cause retries when there are concurrent
read and writes in one cache line.
Tested with IPoIB on PPC64, saw ~5% throughput
improvement.

When sending QP1 MAD packets which use a GRH, the source GID
(which consists of the 64-bit subnet prefix, and the 64 bit port GUID)
must be included in the packet GRH.
For SR-IOV, a GID cache is used, since the source GID needs to be the
slave's source GID, and not the Hypervisor's GID. This cache also
included a subnet_prefix. Unfortunately, the subnet_prefix field in
the cache was never initialized (to the default subnet prefix 0xfe80::0).
As a result, this field remained all zeroes. Therefore, when SR-IOV
was active, all QP1 packets which included a GRH had a source GID
subnet prefix of all-zeroes.
However, the subnet-prefix should initially be 0xfe80::0 (the default
subnet prefix). In addition, if OpenSM modifies a port's subnet prefix,
the new subnet prefix must be used in the GRH when sending QP1 packets.
To fix this we now initialize the subnet prefix in the SR-IOV GID cache
to the default subnet prefix. We update the cached value if/when OpenSM
modifies the port's subnet prefix. We take this cached value when sending
QP1 packets when SR-IOV is active.
Note that the value is stored as an atomic64. This eliminates any need
for locking when the subnet prefix is being updated.
Note also that we depend on the FW generating the "port management change"
event for tracking subnet-prefix changes performed by OpenSM. If running
early FW (before 2.9.4630), subnet prefix changes will not be tracked (but
the default subnet prefix still will be stored in the cache; therefore
users who do not modify the subnet prefix will not have a problem).
IF there is a need for such tracking also for early FW, we will add that
capability in a subsequent patch.

Because of an incorrect bit-masking done on the join state bits, when
handling a join request we failed to detect a difference between the
group join state and the request join state when joining as send only
full member (0x8). This caused the MC join request not to be sent.
This issue is relevant only when SRIOV is enabled and SM supports
send only full member.
This fix separates scope bits and join states bits a nibble each.

This fix solves a race between light flush and on the fly joins.
Light flush doesn't set the device to down and unset IPOIB_OPER_UP
flag, this means that if while flushing we have a MC join in progress
and the QP was attached to BC MGID we can have a mismatches when
re-attaching a QP to the BC MGID.
The light flush would set the broadcast group to NULL causing an on
the fly join to rejoin and reattach to the BC MCG as well as adding
the BC MGID to the multicast list. The flush process would later on
remove the BC MGID and detach it from the QP. On the next flush
the BC MGID is present in the multicast list but not found when trying
to detach it because of the previous double attach and single detach.

The function send_leave sets the member: group->query_id
(group->query_id = ret) after calling the sa_query, but leave_handler
can be executed before the setting and it might delete the group object,
and will get a memory corruption.
Additionally, this patch gets rid of group->query_id variable which is
not used.

When a new CM connection is being requested, ipoib driver copies data
from the path pointer in the CM/tx object, the path object might be
invalid at the point and memory corruption will happened later when now
the CM driver will try using that data.
The next scenario demonstrates it:
neigh_add_path --> ipoib_cm_create_tx -->
queue_work (pointer to path is in the cm/tx struct)
#while the work is still in the queue,
#the port goes down and causes the ipoib_flush_paths:
ipoib_flush_paths --> path_free --> kfree(path)
#at this point the work scheduled starts.
ipoib_cm_tx_start --> copy from the (invalid)path pointer:
(memcpy(&pathrec, &p->path->pathrec, sizeof pathrec);)
-> memory corruption.
To fix that the driver now starts the CM/tx connection only if that
specific path exists in the general paths database.
This check is protected with the relevant locks, and uses the gid from
the neigh member in the CM/tx object which is valid according to the ref
count that was taken by the CM/tx.

Fixes an oops that might happen if uverbs_close races with
remove_one.
Both contexts may run ib_uverbs_cleanup_ucontext, it depends
on the flow.
Currently, there is no protection for a case that remove_one
didn't make the cleanup it runs to its end, the underlying
ib_device was freed then uverbs_close will call
ib_uverbs_cleanup_ucontext and OOPs.
Above might happen if uverbs_close deleted the file from the list
then remove_one didn't find it and runs to its end.
Fixes to protect against that case by a new cleanup lock so that
ib_uverbs_cleanup_ucontext will be called always before that
remove_one is ended.

We are seeing this warning: at net/core/skbuff.c:4174
and before commit a44878d10063 ("IB/ipoib: Use one linear skb in RX flow")
skb truesize was not being set when ipoib was using just one skb.
Removing this line avoids the warning when running tcp tests like iperf.
