commit 9db0ff53cb9b43ed75bacd42a89c1a0ab048b2b0 upstream.
When there is a CM id object that has port assigned to it, it means that
the cm-id asked for the specific port that it should go by it, but if
that port was removed (hot-unplug event) the cm-id was not updated.
In order to fix that the port keeps a list of all the cm-id's that are
planning to go by it, whenever the port is removed it marks all of them
as invalid.
This commit fixes a kernel panic which happens when running traffic between
guests and we force reboot a guest mid traffic, it triggers a kernel panic:

commit 16b0e0695a73b68d8ca40288c8f9614ef208917b upstream.
When creating kernel CQs use 128B CQE stride if the
cache line size is 128B, 64B otherwise. This prevents
multiple CQEs from residing in a 128B cache line,
which can cause retries when there are concurrent
read and writes in one cache line.
Tested with IPoIB on PPC64, saw ~5% throughput
improvement.

commit 8ec07bf8a8b57d6c58927a16a0a22c0115cf2855 upstream.
When sending QP1 MAD packets which use a GRH, the source GID
(which consists of the 64-bit subnet prefix, and the 64 bit port GUID)
must be included in the packet GRH.
For SR-IOV, a GID cache is used, since the source GID needs to be the
slave's source GID, and not the Hypervisor's GID. This cache also
included a subnet_prefix. Unfortunately, the subnet_prefix field in
the cache was never initialized (to the default subnet prefix 0xfe80::0).
As a result, this field remained all zeroes. Therefore, when SR-IOV
was active, all QP1 packets which included a GRH had a source GID
subnet prefix of all-zeroes.
However, the subnet-prefix should initially be 0xfe80::0 (the default
subnet prefix). In addition, if OpenSM modifies a port's subnet prefix,
the new subnet prefix must be used in the GRH when sending QP1 packets.
To fix this we now initialize the subnet prefix in the SR-IOV GID cache
to the default subnet prefix. We update the cached value if/when OpenSM
modifies the port's subnet prefix. We take this cached value when sending
QP1 packets when SR-IOV is active.
Note that the value is stored as an atomic64. This eliminates any need
for locking when the subnet prefix is being updated.
Note also that we depend on the FW generating the "port management change"
event for tracking subnet-prefix changes performed by OpenSM. If running
early FW (before 2.9.4630), subnet prefix changes will not be tracked (but
the default subnet prefix still will be stored in the cache; therefore
users who do not modify the subnet prefix will not have a problem).
IF there is a need for such tracking also for early FW, we will add that
capability in a subsequent patch.

commit e5ac40cd66c2f3cd11bc5edc658f012661b16347 upstream.
Because of an incorrect bit-masking done on the join state bits, when
handling a join request we failed to detect a difference between the
group join state and the request join state when joining as send only
full member (0x8). This caused the MC join request not to be sent.
This issue is relevant only when SRIOV is enabled and SM supports
send only full member.
This fix separates scope bits and join states bits a nibble each.

commit 344bacca8cd811809fc33a249f2738ab757d327f upstream.
This fix solves a race between light flush and on the fly joins.
Light flush doesn't set the device to down and unset IPOIB_OPER_UP
flag, this means that if while flushing we have a MC join in progress
and the QP was attached to BC MGID we can have a mismatches when
re-attaching a QP to the BC MGID.
The light flush would set the broadcast group to NULL causing an on
the fly join to rejoin and reattach to the BC MCG as well as adding
the BC MGID to the multicast list. The flush process would later on
remove the BC MGID and detach it from the QP. On the next flush
the BC MGID is present in the multicast list but not found when trying
to detach it because of the previous double attach and single detach.

commit 68c6bcdd8bd00394c234b915ab9b97c74104130c upstream.
The function send_leave sets the member: group->query_id
(group->query_id = ret) after calling the sa_query, but leave_handler
can be executed before the setting and it might delete the group object,
and will get a memory corruption.

commit 546481c2816ea3c061ee9d5658eb48070f69212e upstream.
When a new CM connection is being requested, ipoib driver copies data
from the path pointer in the CM/tx object, the path object might be
invalid at the point and memory corruption will happened later when now
the CM driver will try using that data.
The next scenario demonstrates it:
neigh_add_path --> ipoib_cm_create_tx -->
queue_work (pointer to path is in the cm/tx struct)
#while the work is still in the queue,
#the port goes down and causes the ipoib_flush_paths:
ipoib_flush_paths --> path_free --> kfree(path)
#at this point the work scheduled starts.
ipoib_cm_tx_start --> copy from the (invalid)path pointer:
(memcpy(&pathrec, &p->path->pathrec, sizeof pathrec);)
-> memory corruption.
To fix that the driver now starts the CM/tx connection only if that
specific path exists in the general paths database.
This check is protected with the relevant locks, and uses the gid from
the neigh member in the CM/tx object which is valid according to the ref
count that was taken by the CM/tx.

commit d1e09f304a1d9651c5059ebfeb696dc2effc9b32 upstream.
Fixes an oops that might happen if uverbs_close races with
remove_one.
Both contexts may run ib_uverbs_cleanup_ucontext, it depends
on the flow.
Currently, there is no protection for a case that remove_one
didn't make the cleanup it runs to its end, the underlying
ib_device was freed then uverbs_close will call
ib_uverbs_cleanup_ucontext and OOPs.
Above might happen if uverbs_close deleted the file from the list
then remove_one didn't find it and runs to its end.
Fixes to protect against that case by a new cleanup lock so that
ib_uverbs_cleanup_ucontext will be called always before that
remove_one is ended.

commit a6100603a4a87fc436199362bdb81cb849faaf6e upstream.
Fix mad send error flow to prevent double freeing address handles,
and leaking tx_ring entries when SRIOV is active.
If ib_mad_post_send fails, the address handle pointer in the tx_ring entry

incremented (or there will be a leak of tx_ring entries).

commit 2788cf3bd90af3791c3195c52391bcf34fa67b40 upstream.
FW port-change events are fired on Active <-> non Active port state
transitions only.
When the port state changes from Active to Initializing (Active ->
Down -> Initializing), a single event is fired.
The HCA transitions from Down to Initializing unless prevented from
doing so, hence the driver should also propagate events when the port
state is Initializing to consumers so they'll be aware that the port
is no longer Active and act accordingly.

commit c9b254955b9f8814966f5dabd34c39d0e0a2b437 upstream.
If the caller specified IB_SEND_FENCE in the send flags of the work
request and no previous work request stated that the successive one
should be fenced, the work request would be executed without a fence.
This could result in RDMA read or atomic operations failure due to a MR
being invalidated. Fix this by adding the mlx5 enumeration for fencing
RDMA/atomic operations and fix the logic to apply this.

commit 8c5122e45a10a9262f872b53f151a592e870f905 upstream.
When this code was reworked for IBoE support the order of assignments
for the sl_tclass_flowlabel got flipped around resulting in
TClass & FlowLabel being permanently set to 0 in the packet headers.
This breaks IB routers that rely on these headers, but only affects
kernel users - libmlx4 does this properly for user space.

commit 32cc92c7b5e52357a0a24010bae9eb257fa75d3e upstream.
For T4, kernel mode qps don't use the user doorbell. User mode qps during
flow control db ringing are forced into kernel, where user doorbell is
treated as kernel doorbell and proper bar2 offset in bar2 virtual space is
calculated, which incase of T4 is a bogus address, causing a kernel panic
due to illegal write during doorbell ringing.
In case of T4, kernel mode qp bar2 virtual address should be 0. Added T4
check during bar2 virtual address calculation to return 0. Fixed Bar2
range checks based on bar2 physical address.

commit e6bd18f57aad1a2d1ef40e646d03ed0f2515c9e3 upstream.
The drivers/infiniband stack uses write() as a replacement for
bi-directional ioctl(). This is not safe. There are ways to
trigger write calls that result in the return structure that
is normally written to user space being shunted off to user
specified kernel memory instead.
For the immediate repair, detect and deny suspicious accesses to
the write API.
For long term, update the user space libraries and the kernel API
to something that doesn't present the same security vulnerabilities
(likely a structured ioctl() interface).
The impacted uAPI interfaces are generally only available if
hardware from drivers/infiniband is installed in the system.

commit 986ef95ecdd3eb6fa29433e68faa94c7624083be upstream.
mlx5 devices (Connect-IB, ConnectX-4, ConnectX-4-LX) has a limitation
where rdma read work queue entries cannot exceed 512 bytes.
A rdma_read wqe needs to fit in 512 bytes:
- wqe control segment (16 bytes)
- rdma segment (16 bytes)
- scatter elements (16 bytes each)
So max_sge_rd should be: (512 - 16 - 16) / 16 = 30.

commit 6d1fba0c2cc7efe42fd761ecbba833ed0ea7b07e upstream.
When we receive an event that triggers connection termination,
we have a a couple of things we may want to do:
1. In case we are already terminating, bailout early
2. In case we are connected but not bound, disconnect and schedule
a connection cleanup silently (don't reinstate)
3. In case we are connected and bound, disconnect and reinstate the connection
This rework fixes a bug that was detected against a mis-behaved
initiator which rejected our rdma_cm accept, in this stage the
isert_conn is no bound and reinstate caused a bogus dereference.
What's great about this is that we don't need the
post_recv_buf_count anymore, so get rid of it.

commit 51093254bf879bc9ce96590400a87897c7498463 upstream.
Let the target core check task existence instead of the SRP target
driver. Additionally, let the target core check the validity of the
task management request instead of the ib_srpt driver.

commit 649367735ee5dedb128d9fac0b86ba7e0fe7ae3b upstream.
cma_validate_port wrongly assumed that Ethernet devices are RoCE
devices and thus their ndev should be matched in the GID table.
This broke the iWarp support. Fixing that matching the ndev only if
we work on a RoCE port.

commit fbbeb8632bf0b46ab44cfcedc4654cd7831b7161 upstream.
The current code is problematic when the QP creation and ipoib is used to
support NFS and NFS desires to do IO for paging purposes. In that case, the
GFP_KERNEL allocation in qib_qp.c causes a deadlock in tight memory
situations.
This fix adds support to create queue pair with GFP_NOIO flag for connected
mode only to cleanly fail the create queue pair in those situations.

commit 4bfdf635c668869c69fd18ece37ec66fb6f38fcf upstream.
ib_send_cm_drep() calls cm_enter_timewait() while holding a spinlock
that can be locked from inside an interrupt handler. Hence do not
enable interrupts inside cm_enter_timewait() if called with interrupts
disabled.
This patch fixes e.g. the following deadlock:

Recently Dough Ledford reported a deadlock happening
between ocrdma-load sequence and NetworkManager service
issuing "open" on be2net interface.
The deadlock happens when any be2net hook (e.g. open/close) is called
in parallel to insmod ocrdma.ko.
A. be2net is sending administrative open/close event to ocrdma holding
device_list_mutex. It does this from ndo_open/ndo_stop hooks of be2net.
So sequence of locks is rtnl_lock---> device_list lock
B. When new ocrdma roce device gets registered, infiniband stack now
takes rtnl_lock in ib_register_device() in GID initialization routines.
So sequence of locks in this path is device_list lock ---> rtnl_lock.
This improper locking sequence causes deadlock.
With this patch we stop using administrative open and close events
injected by be2net driver. These events were used to dispatch PORT_ACTIVE
and PORT_ERROR events to the IB-stack. This patch implements a logic
to receive async-link-events generated from CNA whenever link-state-change
is detected. Now on, these async-events will be used to dispatch
PORT_ACTIVE and PORT_ERROR events to IB-stack.
Depending on async-events from CNA removes the need to hold device-list-mutex
and thus breaks the busy-wait scenario.
