Springville/i211 with a blank Flash/iNVM use different PCI IDs. Extend
the driver to load despite i210/i211 data sheets claiming tools only,
not for driver.
Only warn rather than fail on NVM validation failures on Apalis T30.
Revise Ethernet MAC address assignment: should now handle up to two
instances of custom user MACs (2nd one with a 0x100000 offset). This
way customer does not have to worry about NVM on a secondary Ethernet
on the carrier board and still gets a valid official MAC address from
us (e.g. analogous to how we did it on our Protea carrier board).
Use the Toradex OUI as default MAC address if no valid one is
encountered.
Tested on samples of Apalis T30 2GB V1.0B, V1.0C, V1.1A, Apalis T30 1GB
V1.0A, V1.1A and Apalis T30 1GB IT V1.1A both with blank NVMs as well
as iNVMs programmed with Intel's defaults.

I211 with a blank iNVM uses a different PCI ID. Hack the driver to load
despite i211 data sheet claiming tools only, not for driver.
Please note that the existing driver hacks concerning NVM validation
skipping and Ethernet MAC address assignment equally apply.

after review of all intel drivers, found several instances where
drivers had the incorrect pattern of:
memory mapped write();
delay();
which should always be:
memory mapped write();
write flush(); /* aka memory mapped read */
delay();
explanation:
The reason for including the flush is that writes can be held
(posted) in PCI/PCIe bridges, but the read always has to complete
synchronously and therefore has to flush all pending writes to a
device. If a write is held and followed by a delay, the delay
means nothing because the write may not have reached hardware
(maybe even not until the next read)

Register writes followed by a delay are required to have a flush
before the delay in order to commit the values to the register. Without
the flush, the code following the delay may not function correctly.

Private rx_csum flags are now duplicate of netdev->features & NETIF_F_RXCSUM.
Removing this needs deeper surgery.
Things noticed:
- HW VLAN acceleration probably can be toggled, but it's left as is
- the resets on RX csum offload change can probably be avoided
- there is A LOT of copy-and-pasted code here

When SR-IOV is enabled, i350 devices fail to pass traffic. This is due to
the driver attempting to enable RSS on the PF device, which is not
supported by the i350.
When max_vfs is specified on an i350 adapter, set the number of RSS queues
to 1.
This issue affects 2.6.39 as well.

After discovering that wide use of prefetch on modern CPUs
could be a net loss instead of a win, net drivers which were
relying on the implicit inclusion of prefetch.h via the list
headers showed up in the resulting cleanup fallout. Give
them an explicit include via the following $0.02 script.
=========================================
#!/bin/bash

grep -q '<linux/prefetch.h>' $i

echo '#include <linux/prefetch.h>'
echo .
echo w
echo q
) | ed -s $i > /dev/null 2>&1
if [ $? != 0 ]; then
echo $i needs manual fixup

This updates the network drivers so that they don't access the
ethtool_cmd::speed field directly, but use ethtool_cmd_speed()
instead.
For most of the drivers, these changes are purely cosmetic and don't
fix any problem, such as for those 1GbE/10GbE drivers that indirectly
call their own ethtool get_settings()/mii_ethtool_gset(). The changes
are meant to enforce code consistency and provide robustness with
future larger throughputs, at the expense of a few CPU cycles for each
ethtool operation.
All drivers compiled with make allyesconfig ion x86_64 have been
updated.

Implemented igb_ndo_set_vf_bw function which is being used
by iproute2 tool. In addition, updated igb_ndo_get_vf_config function
to show the actual rate limit to the user.
The rate limitation can be configured only when the link is up.
The rate limit value can be ranged between 0 and actual
link speed measured in Mbps. A value of '0' disables the rate limit for
this specific VF.
iproute2 usage will be 'ip link set ethX vf Y rate Z'.
After the command is made, the rate will be changed instantly.
To view the current rate limit, use 'ip link show ethX'.
The rates will be zeroed only upon driver reload or a link speed change.
This feature is being supported only by 82576 device.
