Add barrier() to bnx2_get_hw_{tx|rx}_cons() to fix this issue:

This issue was reported by multiple i386 users. Without barrier(),
the compiled code looks like the following where %eax contains the
address of the tx_cons or rx_cons in the DMA status block. The
status block contents can change between the cmpb and the movzwl
instruction. The driver would crash if the value was not 0xff during
the cmpb instruction, but changed to 0xff during the movzwl
instruction.

We need to be symmetrical in what is done when key is set and cleared.
This is important wrt the key flags as they are used during key
clearing and if they are not set when the key is set the key cannot be
cleared completely.
This addresses the many occurences of the WARN found in
iwl_set_tkip_dynamic_key_info() and tracked in
http://www.kerneloops.org/searchweek.php?search=iwl_set_dynamic_key
If calling iwl_set_tkip_dynamic_key_info()/iwl_remove_dynamic_key()
pair a few times in a row will cause that we run out of key space.
This is because the index stored in the key flags is used by
iwl_remove_dynamic_key() to decide if it should remove the key.
Unfortunately the key flags, and hence the key index is currently only
set at the time the key is written to the device (in
iwl_update_tkip_key()) and _not_ in iwl_set_tkip_dynamic_key_info().
Fix this by setting flags in iwl_set_tkip_dynamic_key_info().

This fix is only needed for 2.6.29.y tree, since in 2.6.30 and later IGB
has moved to using GRO instead of LRO.
igb supports LRO, but was not setting any hooks to the ->set_flags
ethtool_ops function. This would trigger warnings if the user tried
to enable or disable LRO.

Patch to fix bad length checking in e1000. E1000 by default does two
things:
1) Spans rx descriptors for packets that don't fit into 1 skb on recieve
2) Strips the crc from a frame by subtracting 4 bytes from the length prior to
doing an skb_put
Since the e1000 driver isn't written to support receiving packets that span
multiple rx buffers, it checks the End of Packet bit of every frame, and
discards it if its not set. This places us in a situation where, if we have a
spanning packet, the first part is discarded, but the second part is not (since
it is the end of packet, and it passes the EOP bit test). If the second part of
the frame is small (4 bytes or less), we subtract 4 from it to remove its crc,
underflow the length, and wind up in skb_over_panic, when we try to skb_put a
huge number of bytes into the skb. This amounts to a remote DOS attack through
careful selection of frame size in relation to interface MTU. The fix for this
is already in the e1000e driver, as well as the e1000 sourceforge driver, but no
one ever pushed it to e1000. This is lifted straight from e1000e, and prevents
small frames from causing the underflow described above

The 8169 chip only generates MSI interrupts when all enabled event
sources are quiescent and one or more sources transition to active. If
not all of the active events are acknowledged, or a new event becomes
active while the existing ones are cleared in the handler, we will not
see a new interrupt.
The current interrupt handler masks off the Rx and Tx events once the
NAPI handler has been scheduled, which opens a race window in which we
can get another Rx or Tx event and never ACK'ing it, stopping all
activity until the link is reset (ifconfig down/up). Fix this by always
ACK'ing all event sources, and loop in the handler until we have all
sources quiescent.

Changeset ca17584bf2ad1b1e37a5c0e4386728cc5fc9dabc ("mac8390: update
to net_device_ops") broke mac8390 by adding 8390.o to the link. That
meant that lib8390.c was included twice, once in mac8390.c and once in
8390.c, subject to different macros. This patch reverts that by
avoiding the wrappers in 8390.c. They seem to be of no value since
COMPAT_NET_DEV_OPS is going away soon.
Tested with a Kinetics EtherPort card.

[STABLE] backport upstream commit e151a60ad1faffb6241cf7eb6846353df1f33a32
a recent fix to e1000 (commit 15b2bee2) caused KVM/QEMU/VMware based
virtualized e1000 interfaces to begin failing when resetting.
This is because the driver in a virtual environment doesn't
get to run instructions *AT ALL* when an interrupt is asserted.
The interrupt code runs immediately and this recent bug fix
allows an interrupt to be possible when the interrupt handler
will reject it (due to the new code), when being called from
any path in the driver that holds the E1000_RESETTING flag.
the driver should use the __E1000_DOWN flag instead of the
__E1000_RESETTING flag to prevent interrupt execution
while reconfiguring the hardware.

rndis_wext_link_change() might be called from rndis_command() at
initialization stage and priv->workqueue/priv->work have not been
initialized yet. This causes invalid opcode at rndis_wext_bind on
some brands of bcm4320.
Fix by initializing workqueue/workers in rndis_wext_bind() before
rndis_command is used.

Currently, when OOM occurs during rx ring refill, mv643xx_eth will get
into an infinite loop, due to the refill function setting the OOM bit
but not clearing the 'rx refill needed' bit for this queue, while the
calling function (the NAPI poll handler) will call the refill function
in a loop until the 'rx refill needed' bit goes off, without checking
the OOM bit.
This patch fixes this by checking the OOM bit in the NAPI poll handler
before attempting to do rx refill. This means that once OOM occurs,
we won't try to do any memory allocations again until the next invocation
of the poll handler.
While we're at it, change the OOM flag to be a single bit instead of
one bit per receive queue since OOM is a system state rather than a
per-queue state, and cancel the OOM timer on entry to the NAPI poll
handler if it's running to prevent it from firing when we've already
come out of OOM.

On several mv643xx_eth hardware versions, the two 64bit mib counters
for 'good octets received' and 'good octets sent' are actually 32bit
counters, and reading from the upper half of the register has the same
effect as reading from the lower half of the register: an atomic
read-and-clear of the entire 32bit counter value. This can under heavy
traffic occasionally lead to small numbers being added to the upper
half of the 64bit mib counter even though no 32bit wrap has occured.
Since we poll the mib counters at least every 30 seconds anyway, we
might as well just skip the reads of the upper halves of the hardware
counters without breaking the stats, which this patch does.

This patch adds poisoning and sanity checking to the RX DMA buffers.
This is used for protection against buggy hardware/firmware that raises
RX interrupts without doing an actual DMA transfer.
This mechanism protects against rare "bad packets" (due to uninitialized skb data)
and rare kernel crashes due to uninitialized RX headers.
The poison is selected to not match on valid frames and to be cheap for checking.
The poison check mechanism _might_ trigger incorrectly, if we are voluntarily
receiving frames with bad PLCP headers. However, this is nonfatal, because the
chance of such a match is basically zero and in case it happens it just results
in dropping the packet.
Bad-PLCP RX defaults to off, and you should leave it off unless you want to listen
to the latest news broadcasted by your microwave oven.
This patch also moves the initialization of the RX-header "length" field in front of
the mapping of the DMA buffer. The CPU should not touch the buffer after we mapped it.

Fix a zero address hole bug in the bonding arp_ip_target list
that was causing the bond to ignore ARP replies (bugz 13006).
Instead of just setting the array entry to zero, we now
copy any additional entries down one slot, putting the
zero entry at the end. With this change we can now have
all the loops that walk the array stop when they hit a zero
since there will be no addresses after it.

The BUG_ON(skge->tx_ring.to_use != skge->tx_ring.to_clean) in skge_up()
was sometimes observed when setting MTU.
skge_down() disables the TX queue, but then reenables it by mistake via
skge_tx_clean().
Fix it by moving the waking of the queue from skge_tx_clean() to the
other caller. And to make sure start_xmit is not in progress on another
CPU, skge_down() should call netif_tx_disable().
The bug was reported to me by Jiri Jilek whose Debian system sometimes
failed to boot. He tested the patch and the bug did not happen anymore.

On a MSI MS-6702E mainboard, when in rtl8169_init_one() for the first time
after BIOS has run, IntrStatus reads 5 after chip has been reset.
IntrStatus should equal 0 there, so patch changes IntrStatus reset to happen
after chip reset instead of before.
Remark (Francois):
Assuming that the loglevel of the driver is increased above NETIF_MSG_INTR,
the bug reveals itself with a typical "interrupt 0025 in poll" message
at startup. In retrospect, the message should had been read as an hint of
an unexpected hardware state several months ago :o(

At rmmod stage, the code path is the following one :
rt2x00lib_remove_dev
-> rt2x00lib_uninitialize()
-> rt2x00rfkill_unregister()
-> rfkill_unregister()
-> rt2x00rfkill_free()
-> rfkill_free()
The problem is that rfkill_free should not be called after rfkill_register
otherwise put_device(&rfkill->dev) will be called 2 times. This patch
fixes this by only calling rt2x00rfkill_free() when rt2x00rfkill_register()
hasn't been called or has failed.
This patch is for 2.6.29 only. The code in question has completely disappeared
in 2.6.30 and does not contain this bug.

ath5k sets up a mapping table from the hardware rate index to
the rate index used by mac80211; however, we have seen some
received frames with incorrect rate indexes. Such frames
normally get dropped with a warning in __ieee80211_rx(),
but it doesn't include enough information to track down the
error.
This patch adds a warning to hw_to_driver_rix for any lookups
that result in a rate index of -1, then returns a valid rate so
the frame can be processed.

The MIB interrupt fires whenever counters overflow; however without
support for automatic noise immunity, we can sometimes get an interrupt
storm. The get_stats() callback reads the counters anyway so we can
disable the interrupt for now until ANI is implemented. This fixes
the issue reported in http://bugzilla.kernel.org/show_bug.cgi?id=12647.

This patch fixes the return type of b43_plcp_get_bitrate_idx_ofdm. If
the plcp contains an error, the function return value is 255 instead
of -1, and the packet was not dropped. This causes a warning in

ath5k_reset can be called from process context, which in turn can
call ath5k_beacon_config which takes the sc->block spinlock. Since
it can also be taken in hard irq context, use spin_lock_irqsave
everywhere. This fixes a potential deadlock in adhoc mode.

This is not a fatal message, hitting it simply means we're
going to tell the upper layers to slow their horses down but
as we make more descriptors available we let the show continue
by waking up the queues in ath_wake_mac80211_queue().
We downgrade this as otherwise we fill up your kernel log with
messages which can be common under heavy traffic.

This patch fixes the issue by removing offending (and somewhat
duplicate) code from init_phy() routine, and changes _probe()
function to use uec_mdio_bus_name().
Also, since we fully construct phy_bus_id in the _probe() routine,
we no longer need ->phy_address and ->mdio_bus fields in
ucc_geth_info structure.
I wish the patch would be a bit shorter, but it seems like the only
way to fix the issue in a sane way. Luckily, the patch has been
tested with real PHYs and fixed-link, so no further regressions
expected.

This fixes a locking bug in the dm9000 driver. It calls
request_irq() without setting IRQF_DISABLED ... which is
correct for handlers that support IRQ sharing, since that
behavior is not guaranteed for shared IRQs. However, its
IRQ handler then wrongly assumes that IRQs are blocked.
So the fix just uses the right spinlock primitives in the
IRQ handler.
NOTE: this is a classic example of the type of bug which
lockdep currently masks by forcibly setting IRQF_DISABLED
on IRQ handlers that did not request that flag.

smsc911x_phy_adjust_link is called periodically by the phy layer (as
it's run in polling mode), and it only updates the hardware when it sees
a change in duplex or carrier. This patch clears the last known values
every time the interface is brought up, instead of only when the module
is loaded.
Without this patch the adjust_link function never updates the hardware
after an ifconfig down; ifconfig up. On a full duplex link this causes
the tx error counter to increment, even though packets are correctly
transmitted, as the default MAC_CR register setting is for half duplex.
The tx errors are "no carrier" errors, which should be ignored in
full-duplex mode. When MAC_CR is set to "full duplex" mode they are
correctly ignored by the hardware.
Note that even with this patch the tx error counter can increment if
packets are transmitted between "ifconfig up" and the first phy poll
interval. An improved solution would use the phy interrupt with phylib,
but I haven't managed to make this work 100% robustly yet.

Tulip is currently doing request_irq before it has done its
initialization. This is usually not a problem because it hasn't
enable interrupts yet, but with DEBUG_SHIRQ on, we call the irq handler
when registering the interrupt as a sanity check.
This can result in a NULL ptr dereference, so call tulip_init_ring
before request_irq, and add a free_ring function to do the freeing
now shared with tulip_close.
Tested with a shell loop running ifup, ifdown in a loop a few hundred
times with DEBUG_SHIRQ on.

Impact: Make NetworkManager work with virtio_net
For now the semantics are simple: There is always carrier.
This allows a seamless experience with e.g., qemu/kvm
where NetworkManager just configures and sets up
everything automagically.
If/when a generally agreed-upon way to control
carrier on/off in the emulator/hypervisor level
emerges, it will be trivial to extend the driver
to support that too, but for now even this 2-liner
makes user experience that much better.

bonding: refactor mii monitor
The un-refactored code checked the link speed and duplex of
every slave on every pass; the refactored code did not do so.
The 802.3ad and balance-alb/tlb modes utilize the speed and
duplex information, and require it to be kept up to date. This patch
adds a notifier check to perform the appropriate updating when the slave
device speed changes.

The MSI-X handler was chosen before the call to pci_enable_msix().
If MSI-X was not available, the wrong MSI-X handler would be used in
INTA mode. This would cause a screaming interrupt problem because
INTA would not be cleared by the MSI-X handler.
Fixed by assigning MSI-X handler after pci_enable_msix() returns
successfully. Also update version to 1.9.3.
