bnx2x: VF RSS support - PF side
changed the configuration of the doorbell HW and it broke iSCSI and FCoE.
We fix this by making compatible changes to the doorbell address in bnx2i
and bnx2fc. For the userspace driver, we need to pass a modified CID
so that the existing userspace driver will calculate the correct doorbell
address and continue to work.

When IO is successfully completed while an abort is pending, eh_abort
incorrectly assumes that abort failed and performes recovery by issuing
cleanup. Howerver, cleanup timesout as the firmware has no clue about
this IO. Fix this by checking if the IO has already completed.

The firmware supports a maximum of 4K FCoE exchanges. In 4-port devices,
or when working in multi-function mode, this resource needs to be distributed
between the various possible FCoE functions.
This information needs to be calculated by bnx2x and propagated into bnx2fc
via cnic. bnx2fc can then use this value to calculate corresponding xid
resources instead of using global constants.

Currently libfcoe is doing some libfc discovery layer initialization outside of
libfc. This patch moves this code into libfc and sets up a split in discovery
(one time) initialization code and (re-configurable) settings that will come in
the next patch.

The fcoemon userspace daemon is searching for the a hostX
under the the /sys/bus/fcoe/devices/ctlrX/ entries. When
interfaces created using fcoe_sysfs and fcoe.ko this linkage
is setup correctly, but bnx2fc is not doing the same thing
and therefore fcoemon does not create the fcoe interface
for bnx2fc.
This patch sets up the correct linkage for bnx2fc such that
fcoemon will work correctly with fcoe_sysfs and bnx2fc.

Delete successive tests to the same location. The code tested the result
of a previous allocation, that itself was already tested. It is changed to
test the result of the most recent allocation.

We used to map doorbell register after FW enable request is complete.
This causes a race condition when unsolicited event is received, and FW
sends a CQE for it. Since the doorbell is not mapped, driver does not
arm CQ, which means FW will not notify the driver for further CQ
completions. To resolve this, map the doorbell between offload and
enable, so that driver is ready to receive the unsolicited packets and
arm the CQ as soon as FW enable is performed.

Software FCoE target always advertises RETRY bit even when there are no
tape LUNs behind the target. This causes the driver to enable FW support
for sequence level error recovery and perform REC/SRR. This patch
arrests the behavior by not enabling SLER feature for this target.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

This patch adds support for the new fcoe_sysfs
control interface to bnx2fc.ko. It keeps the deprecated
interface in tact and therefore either the legacy
or the new control interfaces can be used. A mixed mode
is not supported. A user must either use the new
interfaces or the old ones, but not both.
The fcoe_ctlr's link state is now driven by both the
netdev link state as well as the fcoe_ctlr_device's
enabled attribute. The link must be up and the
fcoe_ctlr_device must be enabled before the FCoE
Controller starts discovery or login.

This patch does a few things.
1) Makes /sys/bus/fcoe/ctlr_{create,destroy} interfaces.
These interfaces take an <ifname> and will either
create an FCoE Controller or destroy an FCoE
Controller depending on which file is written to.
The new FCoE Controller will start in a DISABLED
state and will not do discovery or login until it
is ENABLED. This pause will allow us to configure
the FCoE Controller before enabling it.
2) Makes the 'mode' attribute of a fcoe_ctlr_device
writale. This allows the user to configure the mode
in which the FCoE Controller will start in when it
is ENABLED.
Possible modes are 'Fabric', or 'VN2VN'.
The default mode for a fcoe_ctlr{,_device} is 'Fabric'.
Drivers must implement the set_fcoe_ctlr_mode routine
to support this feature.
libfcoe offers an exported routine to set a FCoE
Controller's mode. The mode can only be changed
when the FCoE Controller is DISABLED.
This patch also removes the get_fcoe_ctlr_mode pointer
in the fcoe_sysfs function template, the code in
fcoe_ctlr.c to get the mode and the assignment of
the fcoe_sysfs function pointer to the fcoe_ctlr.c
implementation (in fcoe and bnx2fc). fcoe_sysfs can
return that value for the mode without consulting the
LLD.
3) Make a 'enabled' attribute of a fcoe_ctlr_device. On a
read, fcoe_sysfs will return the attribute's value. On
a write, fcoe_sysfs will call the LLD (if there is a
callback) to notifiy that the enalbed state has changed.
This patch maintains the old FCoE control interfaces as
module parameters, but it adds comments pointing out that
the old interfaces are deprecated.

gcc 4.8 warns because the memset only clears sizeof(char *) bytes, not

Pull first round of SCSI updates from James Bottomley:
"The most important feature of this patch set is the new async
infrastructure that makes sure async_synchronize_full() synchronizes
all domains and allows us to remove all the hacks (like having
scsi_complete_async_scans() in the device base code) and means that
the async infrastructure will "just work" in future.
The rest is assorted driver updates (aacraid, bnx2fc, virto-scsi,
megaraid, bfa, lpfc, qla2xxx, qla4xxx) plus a lot of infrastructure
work in sas and FC.

During parity errors, the ramrods are not issued to FW. bnx2fc waits for the
timeout value, and proceeds with cleaning up the IOs. Since we are already
out-of-sync with FW, cleanup commands timeout too, and do not get the
completion. This operation takes 36 secs for each session to upload causing
huge delays. To fix this, bnx2fc now gets a PARITY_ERROR from cnic driver, and
upon failure, the driver does not issue any commands to the FW and finishes the
upload process sooner.

bnx2fc had an assumption that the fcoe interface will always start on the vlan
dev. However, some switch implementations (Eg., HP virtual connect FlexFabric)
expects the fcoe interface to be started on physical interface. Do not error
out if the netdev is not a vlan dev.

Deduplication of formats and consolidating tests
makes the object much smaller.
Add bnx2fc_debug.c, add functions for a few logging
functions (BNX2FC_IO_DBG, BNX2FC_TGT_DBG, BNX2FC_HBA_DBG).
Use printf extension %pV.
Add and use pr_fmt and pr_<level>.
Move the debug #include below structure definitions.

The libfc is used by fcoe but fcoe agnostic,
and therefore should not have any fcoe references.
So renaming fcoe_dev_stats from libfc as its for fc_stats.
After that libfc is fcoe string free except some strings for
Open-FCoE.org.

This patch has the SW FCoE driver and the bnx2fc
driver make use of the new fcoe_sysfs API added
earlier in this patch series.
After this patch a fcoe_ctlr_device is allocated with
private data in this order.

libfcoe also takes part in this new model since it
discovers and manages fcoe_fcf instances. The memory
allocation is different for FCFs. I didn't want to
impact libfcoe's fcoe_fcf processing, so this patch
creates fcoe_fcf_device instances for each discovered
fcoe_fcf. The two are paired using a (void * priv)
member of the fcoe_ctlr_device. This allows libfcoe
to continue maintaining its list of fcoe_fcf instances
and simply attaches and detaches them from existing
or new fcoe_fcf_device instances.

Currently the fcoe_ctlr associated with an interface is allocated
as a member of struct bnx2fc_interface. This causes problems when
when later patches attempt to use the new fcoe_sysfs APIs which
allow us to allocate the bnx2fc_interface as private data to a
fcoe_ctlr_device instance. The problem is that libfcoe wants to
be able use pointer math to find a fcoe_ctlr's fcoe_ctlr_device
as well as finding a fcoe_ctlr_device's assocated fcoe_ctlr. To
do this we need to allocate the fcoe_ctlr_device, with private
data for the LLD. The private data will contain the fcoe_ctlr
and its private data will be the bnx2fc_interface.

This prep work will allow us to go from a fcoe_ctlr_device
instance to its fcoe_ctlr as well as from a fcoe_ctlr to its
fcoe_ctlr_device once the fcoe_sysfs API is in use (later
patches in this series).

When the task management IO times out, or a flush operation is performed while
task management IO is pending, driver is not cleaning up the IO. This patch
cleans up the IO for the above cases.

When IO abort times out during eh_abort or a flush operation is performed while
abort is pending, the driver is not cleaning up the IO and thus not reducing
the IO reference count. With this change, as part of explicit logout, the IO is
cleaned up.

Commit 907c07d45199f954ddcf66c2c9763c87d012cb15 added more cases to do FLOGI
retry on receiving bad response. Remove the code that drops the packet and
allow the stack to handle bad FLOGI response.

As with the fcoe sw transport, the bnx2fc packet handler function runs only in
softirq context. Theres no need to disable bottom halves here

Pull networking fixes from David Miller:
1) L2TP doesn't get autoloaded when you try to open an L2TP socket due
to a missing module alias, fix from Benjamin LaHaise.
2) Netlabel and RDS should propagate gfp flags given to them by
callers, fixes from Dan Carpeneter.
3) Recursive locking fix in usbnet wasn't bulletproof and can result in
objects going away mid-flight due to races, fix from Ming Lei.
4) Fix up some confusion about a bool module parameter in netfilter's
iptable_filter and ip6table_filter, from Rusty Russell.
5) If SKB recycling is used via napi_reuse_skb() we end up with
different amounts of headroom reserved than we had at the original
SKB allocation. Fix from Eric Dumazet.
6) Fix races in TG3 driver ring refilling, from Michael Chan.
7) We have callbacks for IPSEC replay notifiers, but some call sites
were not using the ops method and instead were calling one of the
implementations directly. Oops. Fix from Steffen Klassert.
8) Fix IP address validation properly in the bonding driver, the
previous fix only works with netlink where the subnet mask and IP
address are changed in one atomic operation. When 'ifconfig' ioctls
are used the IP address and the subnet mask are changed in two

9) Provide a sky2 module operation to work around power management
