It is suitable for all stable branches up to and including 3.14.* Mikulas commit fd1232b214af43a973443aec6a2808f16ee5bf70 Author: Mikulas Patocka <mpatocka@redhat.com> Date: Tue Apr 8 21:52:05 2014 -0400 sym53c8xx_2: Set DID_REQUEUE return code when aborting squeue This patch fixes I/O errors with the sym53c8xx_2 driver when the disk returns QUEUE FULL status. When the controller encounters an error (including QUEUE FULL or BUSY status), it aborts all not yet submitted requests in the function sym_dequeue_from_squeue. This function aborts them with DID_SOFT_ERROR. If the disk has full tag queue, the request that caused the overflow is aborted with QUEUE FULL status (and the scsi midlayer properly retries it until it is accepted by the disk), but the sym53c8xx_2 driver aborts the following requests with DID_SOFT_ERROR --- for them, the midlayer does just a few retries and then signals the error up to sd. The result is that disk returning QUEUE FULL causes request failures. The error was reproduced on 53c895 with COMPAQ BD03685A24 disk (rebranded ST336607LC) with command queue 48 or 64 tags. The disk has 64 tags, but under some access patterns it return QUEUE FULL when there are less than 64 pending tags. The SCSI specification allows returning QUEUE FULL anytime and it is up to the host to retry. 2014-05-19aacraid: prevent invalid pointer dereferenceMahesh Rajashekhara
It appears that driver runs into a problem here if fibsize is too small because we allocate user_srbcmd with fibsize size only but later we access it until user_srbcmd->sg.count to copy it over to srbcmd. It is not correct to test (fibsize < sizeof(*user_srbcmd)) because this structure already includes one sg element and this is not needed for commands without data. So, we would recommend to add the following (instead of test for fibsize == 0). commit d6532207116307eb7ecbfa7b9e02c53230096a50 upstream. This patch fixes the following kernel panic invoked by uninitialized fields in the chip initialization for the 1G bnx2 iSCSI offload. One of the bits in the chip initialization is being used by the latest firmware to control overflow packets. When this control bit gets enabled erroneously, it would ultimately result in a bad packet placement which would cause the bnx2 driver to dereference a NULL ptr in the placement handler. This can happen under certain stress I/O environment under the Linux iSCSI offload operation. This change only affects Broadcom's 5709 chipset. 2013-06-10fix crash in scsi_dispatch_cmd()James Bottomley
USB surprise removal of sr is triggering an oops in scsi_dispatch_command(). What seems to be happening is that USB is hanging on to a queue reference until the last close of the upper device, so the crash is caused by surprise remove of a mounted CD followed by attempted unmount. The problem is that USB doesn't issue its final commands as part of the SCSI teardown path, but on last close when the block queue is long gone. The long term fix is probably to make sr do the teardown in the same way as sd (so remove all the lower bits on ejection, but keep the upper disk alive until last close of user space). However, the current oops can be simply fixed by not allowing any commands to be sent to a dead queue. commit 57fc2e335fd3c2f898ee73570dc81426c28dc7b4 upstream. Rapid ata hotplug on a libsas controller results in cases where libsas is waiting indefinitely on eh to perform an ata probe. A race exists between scsi_schedule_eh() and scsi_restart_operations() in the case when scsi_restart_operations() issues i/o to other devices in the sas domain. When this happens the host state transitions from SHOST_RECOVERY (set by scsi_schedule_eh) back to SHOST_RUNNING and ->host_busy is non-zero so we put the eh thread to sleep even though ->host_eh_scheduled is active. Before putting the error handler to sleep we need to check if the host_state needs to return to SHOST_RECOVERY for another trip through eh. Since i/o that is released by scsi_restart_operations has been blocked for at least one eh cycle, this implementation allows those i/o's to run before another eh cycle starts to discourage hung task timeouts. The result is random discovery failures depending on configuration. Calls to sas_ex_join_wide_port are the source of the trouble as its return value is errantly assigned to 'res'. Convert it to bool and stop returning its result up the stack. Apparently debian still uses it so fix it and backport to stable before removing it in 3.6. The breakage is caused because the function template in include/scsi/scsi_scan.h is defined to be a nop unless SCSI is built in. That means that in the modular case (which is every distro), the scsi_wait_scan module does a simple async_synchronize_full() instead of waiting for scans. 2012-02-03block: fail SCSI passthrough ioctls on partition devicesPaolo Bonzini

[ Changes with respect to 3.3: return -ENOTTY from scsi_verify_blk_ioctl and -ENOIOCTLCMD from sd_compat_ioctl. ] Linux allows executing the SG_IO ioctl on a partition or LVM volume, and will pass the command to the underlying block device. This is well-known, but it is also a large security problem when (via Unix permissions, ACLs, SELinux or a combination thereof) a program or user needs to be granted access only to part of the disk. This patch lets partitions forward a small set of harmless ioctls; others are logged with printk so that we can see which ioctls are actually sent. In my tests only CDROM_GET_CAPABILITY actually occurred. Of course it was being sent to a (partition on a) hard disk, so it would have failed with ENOTTY and the patch isn't changing anything in practice. Still, I'm treating it specially to avoid spamming the logs. In principle, this restriction should include programs running with CAP_SYS_RAWIO. If for example I let a program access /dev/sda2 and /dev/sdb, it still should not be able to read/write outside the boundaries of /dev/sda2 independent of the capabilities. However, for now programs with CAP_SYS_RAWIO will still be allowed to send the ioctls. Their actions will still be logged. This patch does not affect the non-libata IDE driver. That driver however already tests for bd != bd->bd_contains before issuing some ioctl; it could be restricted further to forbid these ioctls even for programs running with CAP_SYS_ADMIN/CAP_SYS_RAWIO. However the check in scsi_request_fn() is imperfect as it only signals that we _might start_ aborting commands, not that we've actually aborted some. So move the printk inside the scsi_kill_request function, this will also give us a hint about which commands are aborted. The call to complete() in st_scsi_execute_end() wakes up sleeping thread in write_behind_check(), which frees the st_request, thus invalidating the pointer to the associated bio structure, which is then passed to the blk_rq_unmap_user(). Fix by storing pointer to bio structure into temporary local variable. This bug is present since at least linux-2.6.32. 2011-11-07ipr: Always initiate hard reset in kdump kernelAnton Blanchard
commit 5d7c20b7fa5c6ca19e871b4050e321c99d32bd43 upstream. During kdump testing I noticed timeouts when initialising each IPR adapter. While the driver has logic to detect an adapter in an indeterminate state, it wasn't triggering and each adapter went through a 5 minute timeout before finally going operational. Some analysis showed the needs_hard_reset flag wasn't getting set. We can check the reset_devices kernel parameter which is set by kdump and force a full reset. This fixes the problem. In an enclosure model where there are chaining expanders to a large body of storage, it was discovered that libsas, responding to a broadcast event change, would only revalidate the domain of first child expander in the list. The issue is that the pointer value to the discovered source device was used to break out of the loop, rather than the content of the pointer. This still remains non-compliant as the revalidate domain code is supposed to loop through all child expanders, and not stop at the first one it finds that reports a change count. However, the design of this routine does not allow multiple device discoveries and that would be a more complicated set of patches reserved for another day. We are fixing the glaring bug rather than refactoring the code. There's a code path in pmcraid that can be reached via device ioctl that causes all sorts of ugliness, including heap corruption or triggering the OOM killer due to consecutive allocation of large numbers of pages. First, the user can call pmcraid_chr_ioctl(), with a type PMCRAID_PASSTHROUGH_IOCTL. This calls through to pmcraid_ioctl_passthrough(). Next, a pmcraid_passthrough_ioctl_buffer is copied in, and the request_size variable is set to buffer->ioarcb.data_transfer_length, which is an arbitrary 32-bit signed value provided by the user. If a negative value is provided here, bad things can happen. For example, pmcraid_build_passthrough_ioadls() is called with this request_size, which immediately calls pmcraid_alloc_sglist() with a negative size. The resulting math on allocating a scatter list can result in an overflow in the kzalloc() call (if num_elem is 0, the sglist will be smaller than expected), or if num_elem is unexpectedly large the subsequent loop will call alloc_pages() repeatedly, a high number of pages will be allocated and the OOM killer might be invoked. It looks like preventing this value from being negative in pmcraid_ioctl_passthrough() would be sufficient. The root cause of the problem is the fact that after scsi_remove_device() has been called, the queue is fully torn down. This is actually wrong since the queue can be used until the sdev release function is called. Therefore, we add an extra reference to the queue which is released in sdev->release, so the queue always exists. commit 86cbfb5607d4b81b1a993ff689bbd2addd5d3a9b upstream. SCSI uses request_queue->queuedata == NULL as a signal that the queue is dying. We set this state in the sdev release function. However, this allows a small window where we release the last reference but haven't quite got to this stage yet and so something will try to take a reference in scsi_request_fn and oops. It's very rare, but we had a report here, so we're pushing this as a bug fix The actual fix is to set request_queue->queuedata to NULL in scsi_remove_device() before we drop the reference. This causes correct automatic rejects from scsi_request_fn as people who hold additional references try to submit work and prevents anything from getting a new reference to the sdev that way. At two points in handling device ioctls via /dev/mpt2ctl, user-supplied length values are used to copy data from userspace into heap buffers without bounds checking, allowing controllable heap corruption and subsequently privilege escalation. Additionally, user-supplied values are used to determine the size of a copy_to_user() as well as the offset into the buffer to be read, with no bounds checking, allowing users to read arbitrary kernel memory. 2011-05-09pmcraid: reject negative request sizeDan Rosenberg

There's a code path in pmcraid that can be reached via device ioctl that causes all sorts of ugliness, including heap corruption or triggering the OOM killer due to consecutive allocation of large numbers of pages. Not especially relevant from a security perspective, since users must have CAP_SYS_ADMIN to open the character device. First, the user can call pmcraid_chr_ioctl() with a type PMCRAID_PASSTHROUGH_IOCTL. A pmcraid_passthrough_ioctl_buffer is copied in, and the request_size variable is set to buffer->ioarcb.data_transfer_length, which is an arbitrary 32-bit signed value provided by the user. If a negative value is provided here, bad things can happen. For example, pmcraid_build_passthrough_ioadls() is called with this request_size, which immediately calls pmcraid_alloc_sglist() with a negative size. The resulting math on allocating a scatter list can result in an overflow in the kzalloc() call (if num_elem is 0, the sglist will be smaller than expected), or if num_elem is unexpectedly large the subsequent loop will call alloc_pages() repeatedly, a high number of pages will be allocated and the OOM killer might be invoked. Prevent this value from being negative in pmcraid_ioctl_passthrough(). Without a page 7, we can still use the enclosure code as long as we make up numeric names for the slots. Unfortunately, the current code fails to add any devices because the check for page 10 is in the wrong place if we have no page 7. Fix it so that devices show up even if the enclosure has no page 7. commit 4224489f45b503f0a1f1cf310f76dc108f45689a upstream. There was a configuration page timing out during the initial port enable at driver load time. The port enable would fail, and this would result in the driver unloading itself, meanwhile the driver was accessing freed memory in another context resulting in the panic. The fix is to prevent access to freed memory once the driver had issued the diag reset which woke up the sleeping port enable process. The routine _base_reset_handler was reorganized so the last sleeping process woken up was the port_enable.
