This change add support for new dma buffer type(writecombine and cacheable)
which allows user application has more choices for the buffer type. And if
the dma buffer is cacheable, then add flush interfaces to make it cache
coherent when necessary.

The allocated DMA channels via some opened file descriptor is better
to be bound to this descriptor. Since this can avoid some application
to fake a channel id which may be requested by other applications to
request PXP service. And also, this make it easier to release the dma
channel when application exists abnormally or forgets to release it
explicitly.

The cpu_addr field in struct pxp_mem_desc cannot be used
by user application, so it is not necessary to pass this
field data to user. Now the similar field 'virtual' in
struct pxp_buf_obj is used to store the kernel space
virtual addr for allocated dma buffer.

Create pxp_info struct data for each opened device file descriptor.
And bind all the allocated dma buffers to this struct for each opened
file. This makes the dma buffer lists management safer, more effective
and more flexible.

Some definitions used only by PXP device driver should not stay in
pxp_dma.h which is shared by PXP, EPDC and V4L2. So the patch creates
a new header file pxp_device.h to hold these definitions.

This patch fixes the hang and crash issue of MLB SYNC mode in the driver.
The MITB will casue Sabreauto to hang and crash when testing the SYNC mode.
It is because MITB will cause something error on MLB bus when stopping the
SYNC test. The Sabreauto will keep entering error ISR and hang. Since we
don't know the details about MITB, we make drivers provide IO_CTRL
interface to disable the interrupt in SYNC mode.

In current code, the MTD_NANDFLASH is used to represent both the SLC and
MLC. It is confusing to us.
By adding an explicit comment about these two macros, this patch makes it
clear that:
MTD_NANDFLASH : stands for SLC NAND,
MTD_MLCNANDFLASH : stands for MLC NAND (including TLC).

It's ported from v3.5.7, which contains ePxP DMAENGINE driver and
a client driver named pxp_device. This patch also includes the changes:
- use uapi header file
- remove VM_RESERVED since it's deprecated, and drop redundant VM_IO flag
since it's automatically set in remap_pfn_range()
- use <linux/platform_data/dma-imx.h> instead of <mach/dma.h>
- use devm_kzalloc() instead in pxp_probe()
- use macro __KERNEL__ in pxp_dma uapi header to avoid definition conflict

This is porting mxc vout driver from imx_3.5.7 kernel to
imx_3.10 kernel.
* Put the driver in drivers/media/platform/ directory instead
of drivers/media/video/ directory, since the later one is
renamed to the former one in 3.10 kernel.
* Change the ipu-v3.h header file from <mach/ipu-v3.h> to
<linux/ipu-v3.h>.
* Change the mxc_vidioc_s_crop() function's implementation since
the definition of the vidioc_s_crop() function is modified to
make the last argument be constant.
* Set vfl_dir to be VFL_DIR_TX since the ioctrl validity checks
are improved in 3.10 kernel.
* Remove 'defaut y' setting for VIDEO_MXC_OUTPUT and
VIDEO_MXC_IPU_OUTPUT Kconfigs. They may be configured by kernel
default configure or by user.
* Make VIDEO_MXC_OUTPUT Kconfig depend on FB_MXC Kconfig since
we need framebuffers to be rendered.
* Make VIDEO_MXC_IPU_OUTPUT Kconfig be tristate.
* Split <linux/mxc_v4l2.h> header file up into include/linux/
and include/uapi/linux/ directories so that the userspace may
include the mxc_v4l2.h header file.

snd_pcm_uframes_t is defined as unsigned long so it would take
different sizes depending on 32 or 64bit architectures. As we don't
want this ABI incompatibility, and there is no real 64bit user yet,
let's make it the fixed size with __u32.
Also bump the protocol version number to 0.1.2.

The PPC64 people noticed a missing memory barrier and crufty old
comments in the perf ring buffer code. So update all the comments and
add the missing barrier.
When the architecture implements local_t using atomic_long_t there
will be double barriers issued; but short of introducing more
conditional barrier primitives this is the best we can do.

Pavel Roskin reported that DRM_IOCTL_MODE_GETCONNECTOR was overwritting
the 4 bytes beyond the end of its structure with a 32-bit userspace
running on a 64-bit kernel. This is due to the padding gcc inserts as
the drm_mode_get_connector struct includes a u64 and its size is not a
natural multiple of u64s.
64-bit kernel:

Also add a new RADEON_INFO query to check that CP DMA packets are
supported on the compute ring.
CP DMA has been supported since the 3.8 kernel, but due to an oversight
we forgot to teach the CS checker that the CP DMA packet was legal for
the compute ring on Southern Islands GPUs.
This patch fixes a bug where the radeon driver will incorrectly reject a legal
CP DMA packet from user space. I would like to have the patch
backported to stable so that we don't have to require Mesa users to use a
bleeding edge kernel in order to take advantage of this feature which
is already present in the stable kernels (3.8 and newer).

RFC 4443 has defined two additional codes for ICMPv6 type 1 (destination
unreachable) messages:
5 - Source address failed ingress/egress policy
6 - Reject route to destination
Now they are treated as protocol error and icmpv6_err_convert() converts them
to EPROTO.
RFC 4443 says:
"Codes 5 and 6 are more informative subsets of code 1."
Treat codes 5 and 6 as code 1 (EACCES)
Btw, connect() returning -EPROTO confuses firefox, so that fallback to
other/IPv4 addresses does not work:

broke the "linklayer atm" handling.
tc class add ... htb rate X ceil Y linklayer atm
The linklayer setting is implemented by modifying the rate table
which is send to the kernel. No direct parameter were
transferred to the kernel indicating the linklayer setting.
The commit 56b765b79 ("htb: improved accuracy at high rates")
removed the use of the rate table system.
To keep compatible with older iproute2 utils, this patch detects
the linklayer by parsing the rate table. It also supports future
versions of iproute2 to send this linklayer parameter to the
kernel directly. This is done by using the __reserved field in
struct tc_ratespec, to convey the choosen linklayer option, but
only using the lower 4 bits of this field.
Linklayer detection is limited to speeds below 100Mbit/s, because
at high rates the rtab is gets too inaccurate, so bad that
several fields contain the same values, this resembling the ATM
detect. Fields even start to contain "0" time to send, e.g. at
1000Mbit/s sending a 96 bytes packet cost "0", thus the rtab have
been more broken than we first realized.

Commit 18d627113b83 (firewire: prevent dropping of completed iso packet
header data) was intended to be an obvious bug fix, but libdc1394 and
FlyCap2 depend on the old behaviour by ignoring all returned information
and thus not noticing that not all packets have been received yet. The
result was that the video frame buffers would be saved before they
contained the correct data.
Reintroduce the old behaviour for old clients.

"These are mostly fixes. The biggest exceptions are Josef's skinny
extents and Jan Schmidt's code to rebuild our quota indexes if they
get out of sync (or you enable quotas on an existing filesystem).
The skinny extents are off by default because they are a new variation
on the extent allocation tree format. btrfstune -x enables them, and
the new format makes the extent allocation tree about 30% smaller.
I rebased this a few days ago to rework Dave Sterba's crc checks on
the super block, but almost all of these go back to rc6, since I
though 3.9 was due any minute.
The biggest missing fix is the tracepoint bug that was hit late in
3.9. I ran into problems with that in overnight testing and I'm still
tracking it down. I'll definitely have that fixed for rc2."

audit rule additions containing "-F auid!=4294967295" were failing
with EINVAL because of a regression caused by e1760bd.
Apparently some userland audit rule sets want to know if loginuid uid
has been set and are using a test for auid != 4294967295 to determine
that.
In practice that is a horrible way to ask if a value has been set,
because it relies on subtle implementation details and will break
every time the uid implementation in the kernel changes.
So add a clean way to test if the audit loginuid has been set, and
silently convert the old idiom to the cleaner and more comprehensible
new idiom.

If qgroup tracking is out of sync, a rescan operation can be started. It
iterates the complete extent tree and recalculates all qgroup tracking data.
This is an expensive operation and should not be used unless required.
A filesystem under rescan can still be umounted. The rescan continues on the
next mount. Status information is provided with a separate ioctl while a
rescan operation is in progress.

Two new flags are added to allow omitting the stream header and the
end command for btrfs send streams. This is used in cases where you
send multiple snapshots back-to-back in one stream.
This used to be encoded like this (with 2 snapshots in this example):
<stream header> + <sequence of commands> + <end cmd> +
<stream header> + <sequence of commands> + <end cmd> + EOF
The new format (if the two new flags are used) is this one:
<stream header> + <sequence of commands> +
<sequence of commands> + <end cmd>
Note that the currently existing receivers treat <end cmd> only as
an indication that a new <stream header> is following. This means,
you can just skip the sequence <end cmd> <stream header> without
loosing compatibility. As long as an EOF is following, the currently
existing receivers handle the new format (if the two new flags are
used) exactly as the old one.
So what is the benefit of this change? The goal is to be able to use
a single stream (one TCP connection) to multiplex a request/response
handshake plus Btrfs send streams, all in the same stream. In this
case you cannot evaluate an EOF condition as an end of the Btrfs send
stream. You need something else, and the <end cmd> is just perfect
for this purpose.
The summary is:
The format change is driven by the need to send several Btrfs send
streams over a single TCP connections, with the ability for a repeated
request/response handshake in the middle. And this format change does
not break any existing tool, it is completely compatible.
You could compare the old behaviour of the Btrfs send stream to the
one of ftp where you need a seperate request/response channel and
newly opened data transfer channels for each file, while the new
behaviour is more like http using a single stream for everything.

Programs using virtio headers outside of kernel will no longer
build because u16 type does not exist in userspace. All user ABI
must use __u16 typedef instead.
