Pull SCSI updates from James Bottomley:
"Small number of driver updates (mpi3mr, ufs, scsi_debug).
By far the biggest core change (at 4000+ lines) is the sense code
rework to make our sense handling follow the 16 bit defines in the
SCSI standard rather than our split ASC and ASCQ handling which ends
up being confusing and difficult to grep.
The changes should all be functionally equivalent and so far the bots
and AI seem to agree; the code has been incubating in -next since 16
September with one reported issue fixed.
The remaining updates are minor"

Protect the megasas_get_ctrl_info() call in megasas_resume() with
instance->reset_mutex using scoped_guard().
megasas_get_ctrl_info() may release and reacquire instance->reset_mutex.
Hence, calling this function without holding instance->reset_mutex is not
safe.

Damien Le Moal <dlemoal@kernel.org> says:
This patch series defines as macros all ASC/ASCQ combinations present in
https://www.t10.org/lists/asc-num.txt and converts the SCSI core code and
SCSI low level drivers to use these macros instead of hard-coded
ASC/ASCQs. To do this, struct scsi_sense_hdr and struct scsi_failure are
modified to replace the asc and ascq 8-bits fields with a 16-bits
sense_code field.
This cleans up the code in many places and makes it easier, and self
documented, to test sense codes in the case of errors.
Overall, there should be no functional chnages here.
Note: this series currently applies cleanly only to the scsi-staging tree.
It does not apply to Linus tree as commit 6d81700ad7c4 ("ata: libata-scsi:
do not raise UA for storage element depopulation and restoration") creates
a conflict with patch 37. The conflict is trivial to resolve, but this
will create a conflict in linux-next. Could you maybe rebase scsi-staging
on rc2 ?
Also please note that sashiko did comment about several pre-existing
issues for different drivers, but I am not addressing these issues in
this series.

Refactor the megaraid driver to use the scsi_set_sense() function and
replace all hard-coded additional sense codes and additional sense code
qualifiers with the enum values defined in include/scsi/scsi_sense.h. This
helps with code clarity as the sense codes being processed are easier to
test and self-documented.
No functional change intended.

megasas_make_prp_nvme() builds a command's PRP list in cmd->sg_frame, a
DMA pool buffer of instance->max_chain_frame_sz bytes, spending one
entry per NVMe page of the transfer plus one per page of the buffer for
the chain pointer. The loop runs until the transfer is described and
never checks the buffer bound.
max_hw_sectors comes straight from the MDTS the firmware reports for the
drive. On drives with a large MDTS the only thing keeping the list
inside the buffer was the block layer default of 1280 KiB, which needs
320 entries, which fit into a 4 KiB frame as that holds 512. But since
commit 9b8b84879d4a ("block: Increase BLK_DEF_MAX_SECTORS_CAP") that
default is 4 MiB, and such a transfer needs 1025 entries, so the list
runs a full page past the end of the frame:
sd 1:0:1:0: [sdb] tag#630 page boundary ptr_sgl: 0x00000000ba62d13f
BUG: unable to handle page fault for address: ff663bcb81e7c000

#PF: error_code(0x0002) - not-present page
RIP: 0010:megasas_build_and_issue_cmd_fusion+0xeaa/0x1870 [megaraid_sas]
If the page after the frame happens to be mapped, the overrun does not
fault but silently corrupts the neighbouring pool entry, which is
another in-flight command's PRP list.
Cap max_hw_sectors at what the chain frame can describe, less one page
for transfers that do not start on a page boundary and so need one entry
more. This is the megaraid_sas counterpart of commit 04631f55afc5
("scsi: mpt3sas: Limit NVMe request size to 2 MiB"), but derives the
limit from max_chain_frame_sz rather than hardcoding it.

Back in the 2.6.xx days, it was possible to for low-level drivers to set
the shost lock pointer. That is why there is a default lock and a
pointer to the shost lock.
However, support for this has long been removed, so drop
Scsi_Host.default_lock and make Scsi_Host.host_lock as the actual lock.
Getting the address of embedded host_lock structure just requires adding
a fixed offset value to the shost pointer. However, getting the value of
the host_lock pointer requires loading from a fixed offset to the shost
pointer. The latter should be very slightly slower, which is relevant as
this lock is used a lot throughout the core code and drivers.

The megaraid_cmm_register() function has a local copy of mraid_mmadp_t
on the stack that gets copied into the actual structure used at
runtime. When -fsanitize=thread is enabled, this causes the per-function

completion
Add NULL check for scmd_local in the MPI2_FUNCTION_SCSI_IO_REQUEST case
to handle firmware duplicate/stale completions.
When firmware sends a duplicate completion for a command that was
already processed and returned to the pool, the driver accesses NULL
scmd pointer causing a crash.
Timeline of the bug:
1. Command completes normally, megasas_return_cmd_fusion() called
2. This sets cmd->scmd = NULL and clears io_request with memset(..., 0,
...)
3. Firmware sends duplicate/stale completion for same SMID (firmware
bug)
4. Driver processes reply descriptor again
5. Cleared io_request has Function = 0 (MPI2_FUNCTION_SCSI_IO_REQUEST)
6. Switch statement matches SCSI_IO_REQUEST case by accident
7. Accesses megasas_priv(NULL scmd)->status -> crash at offset 0x228
The offset 0x228 = sizeof(struct scsi_cmnd) 0x220 + offsetof(status)
0x8.
This issue was observed on PERC H330 Mini running firmware 25.5.9.0001
after 3+ days of heavy I/O load.
Crash signature:
BUG: unable to handle kernel NULL pointer dereference at 0x228
RIP: complete_cmd_fusion+0x428
Function: megasas_priv(cmd_fusion->scmd)->status
Add defensive check to skip processing when scmd_local is NULL. This
handles duplicate completions from firmware and prevents accessing freed
command structures. The check protects all scmd_local uses in both the
SCSI_IO path and the fallthrough LDIO path.

This converts some of the visually simpler cases that have been split
over multiple lines. I only did the ones that are easy to verify the
resulting diff by having just that final GFP_KERNEL argument on the next
line.
Somebody should probably do a proper coccinelle script for this, but for
me the trivial script actually resulted in an assertion failure in the
middle of the script. I probably had made it a bit _too_ trivial.
So after fighting that far a while I decided to just do some of the
syntactically simpler cases with variations of the previous 'sed'
scripts.
The more syntactically complex multi-line cases would mostly really want
whitespace cleanup anyway.

This was done entirely with mindless brute force, using
git grep -l '\<k[vmz]*alloc_objs*(.*, GFP_KERNEL)' |
xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/'
to convert the new alloc_obj() users that had a simple GFP_KERNEL
argument to just drop that argument.
Note that due to the extreme simplicity of the scripting, any slightly
more complex cases spread over multiple lines would not be triggered:
they definitely exist, but this covers the vast bulk of the cases, and
the resulting diff is also then easier to check automatically.
For the same reason the 'flex' versions will be done as a separate
conversion.

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:
Single allocations: kmalloc(sizeof(TYPE), ...)
are replaced with: kmalloc_obj(TYPE, ...)
Array allocations: kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with: kmalloc_objs(TYPE, COUNT, ...)
Flex array allocations: kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with: kmalloc_flex(*PTR, FAM, COUNT, ...)
(where TYPE may also be *VAR)
The resulting allocations no longer return "void *", instead returning
"TYPE *".

option has been introduced. This warning is enabled by default and can

that changes the return type of the .queuecommand() implementations from
'int' into 'enum scsi_qc_status'. No functionality has been changed.

-Wflex-array-member-not-at-end was introduced in GCC-14, and we are
getting ready to enable it, globally.
Use the new TRAILING_OVERLAP() helper to fix the following warnings:
drivers/scsi/megaraid/megaraid_sas_fusion.h:1153:31: warning: structure containing a flexible array member is not at the end of another structure [-Wflex-array-member-not-at-end]
drivers/scsi/megaraid/megaraid_sas_fusion.h:1198:32: warning: structure containing a flexible array member is not at the end of another structure [-Wflex-array-member-not-at-end]
This helper creates a union between a flexible-array member (FAM) and a
set of MEMBERS that would otherwise follow it --in this case 'struct
MR_LD_SPAN_MAP ldSpanMap[MAX_LOGICAL_DRIVES_DYN]' and 'struct
MR_LD_SPAN_MAP ldSpanMap[MAX_LOGICAL_DRIVES]' in the corresponding
structures.
This overlays the trailing members onto the FAM (struct MR_LD_SPAN_MAP
ldSpanMap[];) while keeping the FAM and the start of MEMBERS aligned.
The static_assert() ensures this alignment remains, and it's
intentionally placed inmediately after the corresponding structures --no
blank line in between.

The names RESERVE and RELEASE are not only used in <scsi/scsi_proto.h> but
also elsewhere in the kernel:
$ git grep -nHE 'define[[:blank:]]*(RESERVE|RELEASE)[[:blank:]]'
drivers/input/joystick/walkera0701.c:13:#define RESERVE 20000
drivers/s390/char/tape_std.h:56:#define RELEASE 0xD4 /* 3420 NOP, 3480 REJECT */
drivers/s390/char/tape_std.h:58:#define RESERVE 0xF4 /* 3420 NOP, 3480 REJECT */
Additionally, while the names of the symbolic constants RESERVE_10 and
RELEASE_10 include the command length, the command length is not included
in the RESERVE and RELEASE names. Address both issues by renaming the
RESERVE and RELEASE constants into RESERVE_6 and RELEASE_6 respectively.

except those in drivers/net/ and Documentation/.
* Change the text "slave_destroy" into "sdev_destroy" in all source

Bart Van Assche <bvanassche@acm.org> says:
Hi Martin,
Multiple SCSI drivers use snprintf() to format a workqueue name before
invoking one of the create*_workqueue() macros. This patch series
simplifies such code by passing the format string and arguments to
alloc_workqueue(). Additionally, the structure members that are only
used as a temporary buffer for formatting workqueue names are
removed. Please consider this patch series for the next merge window.
Thanks,
Bart.

The workqueue maintainer wants to remove the create*_workqueue() macros
because these macros always set the WQ_MEM_RECLAIM flag and because these
only support literal workqueue names. Hence this patch that replaces the
create*_workqueue() invocations with the definition of this macro. The
WQ_MEM_RECLAIM flag has been retained because I think that flag is necessary
for workqueues created by storage drivers. This patch has been generated by
running spatch and git clang-format. spatch has been invoked as follows:
spatch --in-place --sp-file expand-create-workqueue.spatch $(git grep -lEw 'create_(freezable_|singlethread_|)workqueue' */scsi */ufs)
The contents of the expand-create-workqueue.spatch file is as follows:

flexible array
Replace the deprecated[1] use of a 1-element array in struct
MR_HOST_DEVICE_LIST with a modern flexible array.
One binary difference appears in megasas_host_device_list_query():
struct MR_HOST_DEVICE_LIST *ci;
...
ci = instance->host_device_list_buf;
...
memset(ci, 0, sizeof(*ci));
The memset() clears only the non-flexible array fields. Looking at the rest
of the function, this appears to be fine: firmware is using this region to
communicate with the kernel, so it likely never made sense to clear the
first MR_HOST_DEVICE_LIST_ENTRY.

Hi all,
this series converts the SCSI midlayer and LLDDs to use atomic queue
limits API. It is pretty straight forward, except for the mpt3mr

setting limits from unlocked device iteration callbacks.
I will probably defer the (more complicated) ULD changes to the next

plugging series. With that the series could go in through the SCSI
